Abstract

¹è°æ: Ç÷¾×Åõ¼®À» ÇÏ´Â ¸¸¼º½ÅºÎÀü ȯÀÚ´Â ¹Ýº¹µÈ Ç÷°üõÀÚ·Î ÀÎÇØ Á¤¸ÆÀÇ º¸Á¸»óÅ°¡ ÁÁÁö ¾Ê¾Æ¼­ µ¿Á¤¸Æ·ç Á¶¼º¼úÀÌ ¾î·Á¿î °æ¿ì°¡ ¸¹´Ù. ÀÌ¿¡ ÀúÀÚ´Â Ç÷°üõÀÚ µîÀ¸·ÎºÎÅÍ º¸È£µÈ »ó¿Ïµ¿¸Æ ÁÖÀ§ÀÇ µ¿¹ÝÁ¤¸Æ(vena comitantes)À» Á¤¸ÆÃø ¹®ÇÕÀ¸·Î ÀÌ¿ëÇÏ¿© ÀÎÁ¶Ç÷°ü µ¿Á¤¸Æ·ç Á¶¼º¼úÀ» ½ÃÇàÇÏ¿´°í ÀÌÀÇ °³Á¸À², ÇÕº´Áõ µîÀ» ¾Ë¾Æº¸°íÀÚ ÇÏ¿´´Ù.

´ë»ó ¹× ¹æ¹ý: 2006³â 1¿ùºÎÅÍ 2008³â 5¿ù±îÁö ÀüÁÖÁÖ¸§ ºÎÀ§ÀÇ Ç¥ÀçÁ¤¸ÆÀÇ º¸Á¸»óÅ°¡ ºÒ·®ÇÏ¿© ½ÉºÎÁ¤¸ÆÀÎ µ¿¹ÝÁ¤¸ÆÀ» Á¤¸Æ À¯Ãâ·Î·Î ÀÌ¿ëÇÏ¿© ¼ö¼úÇÑ 12¿¹ÀÇ ÀÎÁ¶Ç÷°ü µ¿Á¤¸Æ·ç ȯÀÚ¸¦ ´ë»óÀ¸·Î ÀÓ»óºÐ¼®À» ½ÃÇàÇÏ¿´´Ù. µ¿¸ÆÃø ¹®ÇÕÀº »ó¿Ïµ¿¸Æ¿¡ ´ÜÃø¹®ÇÕÇÏ¿´À¸¸ç ·çÇÁÇüÅ·Π¼ö¼úÇÏ¿´´Ù. ´ë»óȯÀÚÀÇ ³²³àºñ´Â 7£º5ÀÌ°í Æò±Õ¿¬·ÉÀ» 59¡¾17¼¼ÀÌ¸ç ´ç´¢º´À» µ¿¹ÝÇÑ °æ¿ì°¡ 6¿¹, °íÇ÷¾ÐÀ» µ¿¹ÝÇÑ °æ¿ì°¡ 10¿¹¿´´Ù.

°á°ú: ¼ö¼ú ÈÄ °¨¿°À̳ª ÃâÇ÷ µîÀÇ ÇÕº´ÁõÀº ¾ø¾ú°í 5¸íÀÇ È¯ÀÚ¿¡¼­ ¼ú ÈÄ Æò±Õ 3°³¿ù¿¡ ÇùÂøÀÇ ¼Ò°ßÀ» º¸¿´´Ù. 3, 6, 12°³¿ùÀÇ ÀÏÂ÷ °³ÅëÀ²Àº °¢°¢ 75.0%, 65.6%, 52.5%¿´´Ù. ÇùÂøÀÌ ÀÖ¾ú´ø ȯÀڵ鵵 ÁßÀçÀû ½Ã¼ú ÈÄ Ç÷·ù°¡ °³¼±µÇ¾î ÇöÀç±îÁö Ç÷¾×Åõ¼®ÀÌ °¡´ÉÇÏ¿´°í 12°³¿ùÀÇ ÀÌÂ÷ °³ÅëÀ²Àº 100%¿´´Ù.

°á·Ð: ÀÎÁ¶Ç÷°üÀ» ÀÌ¿ëÇÑ µ¿Á¤¸Æ·ç Á¶¼º¼ú½Ã Á¤¸ÆõÀÚ µî¿¡ ÀÇÇØ ¼Õ»óµÇÁö ¾ÊÀº µ¿¹ÝÁ¤¸ÆÀ» ÀÌ¿ëÇÏ¿© Á¤¸Æ¹®ÇÕ ÇÏ¸é ºñ±³Àû ¾çÈ£ÇÑ °³ÅëÀ²À» º¸À̹ǷΠǥÀçÁ¤¸ÆÀÌ Àû´çÄ¡ ¾ÊÀº °æ¿ì ´ëüÇ÷°üÀ¸·Î »ç¿ëÇصµ ¹«¹æÇÒ °ÍÀ¸·Î »ý°¢µÈ´Ù.

ackground:<> Arteriovenous fistula formation is not always easy to perform in hemodialysis patients because of poor preservation of veins due to repeated venipuncture and cannulation. We analyzed the patency rate and complications of prosthetic arteriovenous fistulas using the vena comitantes as a venous outflow in the antecubital fossa, which are protected from venipuncture.

Material and Method: Between January 2006 and June 2008, 12 patients underwent prosthetic arteriovenous fistula formation using the vena comitantes as a venous outflow. Arterial inflow was via the brachial artery and the graft was placed in a loop fashion. The male-to-female ratio was 7£º5 and the mean age was 59¡¾14 years. Six patients had diabetes mellitus and 10 patients had hypertension.

Result: There were no complications, such as a graft infection or bleeding. Five patients showed postoperative stenosis at an average of 3 months. The primary patency rate was 75.0, 65.6, and 52.2% at 3, 6, and 12 months, respectively. All the patients with stenosis were able to continue hemodialysis after intervention therapy. The secondary patency rate was 100% at 12 months.

Conclusion: Creation of a prosthetic arteriovenous fistula using uninjured vena comitantes resulted in a good patency rate and this vein may become a substitute for inappropriate superficial veins. (Korean J Thorac Cardiovasc Surg 2009;42:41-45)
