A Study on the Categorization of Unmet Healthcare Needs Influencing Factors for Older Adults with Disabilities Living in the Community

Abstract
본 연구는 지역사회에 거주하고 있는 고령 장애인의 건강권 및 생존권 보장과 증진을위하여 미충족 의료 영향요인을 포괄적으로 살펴봄으로써 미충족 의료 해결에 필요한구체적인 논의를 제시하고, 나아가 커뮤니티케어의 기반 구축에 시사점을 제공하고자하였다. 이를 위하여 한국보건사회연구원의 2020년도 노인실태조사 자료를 활용하였고, 분석에는 지역사회에 거주하는 만 65세 이상 장애판정을 받은 고령 장애인 총 418 명의 자료가 활용되었다. 미충족 의료에 영향을 미치는 요인들은 Gelberg-Andersen의취약계층 행동모델을 기반으로 선정하였으며, 잠재계층분석을 통해 도시건강형, 도시취약형, 도농건강형 및 농촌취약형의 4개 유형이 도출되었다. 유형별 차이 및 로지스틱회귀분석을 실시하여 4개 유형이 미충족 의료에 미치는 영향을 검증한 결과, 농촌취약형에 비하여 도시건강형, 도농건강형, 도시취약형 순으로 미충족 의료 발생 가능성이현저히 낮은 것으로 확인되었다. 본 연구는 다양한 영향요인들을 고려함으로써 미충족의료의 발생이 노화와 장애의 이중적 위험에 따른 불가피한 결과가 아닌, 환경과 사회적결정요인에 의한 충분치 못한 의료보장의 결과임을 확인하였다는 점에서 의의가 있으며, 향후 고령 장애인을 대상으로 집단 내 이질성을 고려한 후속 연구가 요구된다.
The purpose of this study was to examine the factors that influence unmet healthcare needs to promote the right to health and survival of older adults with disabilities living in the local community. Furthermore, it provides implications for building a foundation for community care. This study used data from the 2020 National Survey on Older Koreans conducted by the Korea Institute for Health And Social Affairs (KIHASA). A total of 418 older adults with disabilities (aged 65+) living in the community were analyzed. Factors influencing unmet healthcare needs were selected based on Gelberg-Andersen’s behavioral model for vulnerable populations, and the differences were considered by categorizing them using a latent class analysis. In addition, as a result of verifying the effect of the four classes derived through logistic regression on unmet healthcare needs, it was found that there were significant differences by class. The significance of this study is that the occurrence of unmet healthcare needs is not an inevitable result of the double risk of aging and disability, but the result of insufficient healthcare coverage due to environmental and social determinants. A follow-up study considering heterogeneity in the older adults with disabilities is required.
