Abstract

The association of the so-called heavy chain disease protein with a complete
immunoglobulin in a monoclonal gainmopatty has been described in less than 10 cases
and most cases of these show lymphoma like symptoms.
Multiple myeloma with heavy chain disease protein and Bence Jonce protein is
extremly rare plasma cell dyscrasia.
We report a case of which clinical, radiological, bone marrow and protein
electrophoretic study reveals a simple multiple myeloma but immunoelectrophoretic
patterns are different. IEP of serum reveals IgG kappa type immunoglobulin and lambda
type Bence Jonce protein, and IEP of unconcentrated urine also reveals lambda type
Bence Jonce protein. IEP of 300 times concentrated urine reveals gamma heavy chain
protein.
A brief review of current literature is added.
