주사형 전자현미경에 의한 위암의 형태학적 관찰

Abstract
요 약
저자는 위암을 우선 광학현미경 및 투과형 전자현미경에서 분화암과 미분화암으로 구분하
여 SEM상의 형태를 관찰하고 그 차이점을 알아보고자 위 생검으로 위암이 증명된 11예의
위 절제 표본을 광학현미경, 투과형 전자현미경 및 주사형 전자현미경으로 관찰하였다.
광학현미경적으로는 11예 중 7예가 분화암이었으며 병변 주변으로 심한 장화생을 나타내
고, 7예 중 1예는 점막의 표층은 미분화암 형태를 나타냈으며, 미분화암은 4예로 주위의 장
화생은 부분적으로만 나타났으며 4예 중 2예는 표층에만 분화암 형태를 나타냈다. 투과형
전자현미경적으로는 분화암과 미분화암 모두 세포의 자유면은 미융모로 덮혀 있으며, 분화
암은 미융모가 고루 덮여 있는 경우가 많은 반면 미분화암은 미융모가 있는 부분과 없는 부
분이 섞여 나타났으며, 분비과립은 그 수와 밀도가 다양했으며 분화암의 경우 위암 뚜렷하
게 나타났다. 주사형 전자현미경으로는 fold의 발달과 배열의 불규칙성과 개개세포의 다형태
성, 다령 태성 미융모가 주위 점막에 비해 저명하게 나타났다.
분화암은 fold가 잘 유지되어 있어서 fold간의 furrow 가 깊고 널은 반면 미분화암에서는
fold가 작고 불규칙하고 furrow의 형성이 불완전하다. 개개의 암세포는 광학현미경 수준에서
관찰되었던 다형태성, 응집력의 결핍 등이 뚜렷했으며 , 특이한 사항은 다형태성 미융모의
출현이다. 이 다형태성 미융모는 분화암과 미분화암에 공히 나타났으며 그 크기는 평군 0.6
μm와 1, 2㎝이며 투과형 전자현미경상 보는 striated bolder 즉 길고 규칙적인 미융모와 다
형태성 미융모가 섞여 나타나는 것이 흔히 관찰된다. 특히 분화암의 경우는 더욱 뚜렷하다.
그러나 분화도가 중등도인 경우는 주사형 전자현미경으로 구별이 어려웠다. 장화생은 분화
암의 경우 병변 주위로 심하게 나타났으나 미분화암인 경우는 아주 부분적으로 존재했으며,
주사형 전자현미경으로는 표면이 편평하며 주위세포와의 경계가 뚜렷하게 나타났다.
이상의 소견으로 위암은 분화도가 양극단인 경우는 주사형 전자현미경으로 구별이 가능하
나 그 중간형태는 구별이 어려웠으며, 불규칙한 fold, 세포의 다형태성, 다형태성 미융모들은
악성을 시사하는 주요한 소견이었다. 그리고 장화생 세포에서 관찰되었던 striated border가
주사형 전자현미경상 분화암과 미분화암에서 흔히 관찰되고 부분적으로 미융모가 소실된 세
포와 완전히 소실된 세포가 섞여 나타나고, 주변점막에 장화생 세포가 나타남은 분화암과
미분화암이 공통된 전구세포에서 유래했을 가능성을 뒷받침해 주는 소견으로 생각된다.

The author studied 11 cases of adenocarcinoma of the stomach confirmed by
gastrofiberscopic biopsy before in order to differentiate between differentiated and
undifferentiated adenocarcinma by scanning etectron microscope. Light and transmission
electron microscopic examination were done, too. Seven of them are differentiated
accompanied by severe intestinal metaplsia and four of them are undifferentiated with
rearly focal intestinal metaplasia. Two of the undifferentiated cases shows focal tubular
differentiation on the superficial region of the mucosa.
Microvilli on the free border are long, regular on the differentiated type but in state of
variable loss of microvilli under the transmission electron microscope. Number and
density of the mucous granules are variable.
Scanning electron microscopic examination shows prominent disorganization of the
folds, cellular pleomorphism and pleomorphic microvilli are suggestive of early marker of
neoplastic transformation. The size of them are 0.6cm and 1.2 cm on the differentiated
and undifferentiated type respectively.
Disorganization of the folds is an important differential point between differentiated
and undifferentiated type on the lower power examination.
Development of folds, furrow, and hemispheric colliculi are more perminent on the
differentiated adenocarcinoma. Presence of striated border, partial or complete loss of
microvilli and intestinal metaplasia on the undifferentiated and differentiated
adenocarcinomas are consistent with origin from common precursor cells.
