Abstract
림프관종은 소아 연령에 주로 발생하는 림프관계 양성 맥관성 종양으로서 두경부 및 액와
부에 호발한다. 복강에 생기는 경우는 매우 드물지만 장간막, 대망, 결장 장간막의 순으로
보고되어 있다. 한편, 림프관종은 병리학적 특성에 따라 나눌수 있으며, 복강내에서 발생하
는 경우 대부분이 낭성 림프관종으로 알려져 있다. 복강내의 낭성 림프관종은 딴 부위에 비
하여 발생빈도가 상대적으로 드물지만 국내에서도 수례 보고 되어 있다. 이들 림프관종은
임상적으로 개복전에 진단되는 경우가 거의 없고 급성 복증이라는 잠정적 진단하에 개복 수
술을 하는 경우가 대부분이다. 그러나 동일 크기의 다른 종양에 비하여 심하고도 지속적인
통증을 수반하는 이유가 분명히 설명되지 않고 있다. 저자들은 심한 복통을 수반한 복강내
림프관종을 경험하고 복통의 기전을 추정하는데 도움이 되는 조직학적 소견을 중심으로 보
고코자 한다.

A case of cystic lymphangioma of the mesentery with severe and persistent abdominal
pain in a 22-rear-old man is presented. The cyst was filled with chylous fluid.
Microscopically, numerous nerve bundles were incorporated within the lymphangiomatous
walls, and some protruded into the lumen. The above findings lead to a suggestion that
mesenteric lymphangioma may differ from those in the other sites by its abundance of
incorporation of nerve bundles into the lymphangiomatous walls, and that increase of
tumor size by intracystic accumulation of chylous fluid may subsequently result in
increase of intraluminal pressure to compress the nerve bundles with which abdominal
pain is much enhanced.
