Abstract
폐동정맥루(Pulmonary arteriovenous fistula)는 폐모세혈관총의 발달장애로 폐동맥과 폐정
맥간의 비정상적인 연결에 의해 생기는 비교적 드문 질환이다. 이 질환은 1897년 Churton이
처음 기술한 이래 세계적으로 약 400예가 보고되었으나 대부분이 성인예이고 소아에서 발생
하는 경우는 4∼6%에 불과하며 국내문헌에는 소아증례가 기록되어있지 않다. 본 질환은 약
60%의 환자에서 유전성 출혈성 모세혈관확장증(Rendu-Osler-Weber syndrome)과 연관되어
나타나며, 대부분이 단순 흉부X-선상 한개 또는 다수의 결절성 음영으로 발견되나 드물게
양측폐에 미만성으로 모세혈관부위에 동정맥루를 일으키는 수도 있다. 저자들은 8세 소년에
서 생긴 폐동정맥루 1예를 경험하였기에 이를 보고한다.

A case of pulmonary arteriovenous fistula in a 8-year-old boy who presented with
easy fatigability and cyanosis for 4 months, is described. Grossly, there was a large
vascular anomialy measuring 2×2 ㎝ in the center of the removed right lower lobe.
Microscopically, the lesion consisted of dilated arterial and venous structures resembling
a cavernous hemangioma. Subintimal fibrosis and attenuated vascular structure lacking
elastic fiber and representing A-V shunt were also noted in the lesion.
