Abstract
1943년 Uihlein과 McDonald는 충수의 암종을 ① 유암종, ② 복막 위점액종을 야기하는 낭
종형의 점액성 선암종 및 ③ 대장형 선암종으로 분류하였다. 이중 대장형은 육안 및 현미경
소견이 대장 선암종과 같으며 약 3분의 1에서 이미 있던 선종으로부터 생기는 것으로 알려
져 있고, 충수의 해부학적 특성상 내강의 폐색으로 인한 급성 우하복부 동통으로 발현하는
경우가 많다. 그러나 그 빈도는 매우 낮아 소장의 선암종의 빈도에 준하며 예후는 듀크 종
양기 체계와 잘 부합된다. 저자들은 73세 남자에서 발견된 충수의 대장형 유두상 선암종 1
예를 보고한다.

We report a case of colonic type-papillary adenocarcinoma of appendiceal origin in a
73-year-old male patient. The patient presented with right lower quadrant mass and
was operated for a preoperative diagnosis of inflammatory small bowel mass. The mid
one-third of the appendix showed a 3.5×3.3 ㎝ sized, broad-based, intraluminal papillary
mass. Microscopically, it was a well differentiated papillary adenocarcinoma and
infiltrated into the serosa. The tumor cells resembled colonic type adenocarcinoma and
revealed a strong immunoreactivity to carcinoembryonic antigen. Tumor desmoplasia and
acute inflammatory change were prominent.
