Abstract
인체의 스파르가눔증은 한국에서는 드물지 않은 기생충 감염증이다 1917년 Uemura가 한
국인의 하지에서 처음 발견한 이후 다수의 예가 보고되었다. 스파르가눔증은 감염된 뱀이나
개구리를 생식한 경우 plerocercoid 유충이 장을 뚫고 인체의 각부분으로 이행한 후에 증상
을 나타내므로 충체를 중심으로 국소질환을 일으키지만 전신 장기에 병변을 만들기도한다.
대부분의 환자에서는 피하나 연부조직의 작은 종괴로 발견되며 감염 초기에 충체가 장벽을
통과할 때에는 심한 증상이나 증후를 보이지 않는다.
저자들은 만성 복통과 설사를 호소하는 환자에서 장의 부분적 협착이 의심되어 시험적 개
복술을 시행하였던 바, 스파르가눔 감염에 기인하였던 장의 염증성 종괴 1예를 보고한다.

A 39-year-old male patient presented with chronic abdominal pain and intermittent
diarrhea for 8 months. Colon study showed an annular stricture at the ileocecal valve.
He underwent ileocecectomy with Clinical impression of intestinal tuberculosis. The
reseated intestinal wall along the ileocecal junction demonstrated a localized, annular
constriction and intramural nodular inflammatory growth in which were clusters of
multiple microabscesses as well as acute and chronic inflammatory cell infiltration
including eosinophils and fibrosis. Encountered were a few resolving phase of parasitic
granulomatous tunnels in which fragments of degenerated spartanum with foreign body
reaction were found in one focus. He had history of ingesting uncooked frogs 2 rears
auto. The above case suggests that differential diagnosis of inflammatory tumorous
lesions in the intestine should include sparganosis in Korea.
