Abstract
안와에 발생하는 림프계 종양은 비교적 드물며, 때때로 양성 림프계 증식과 악성 림프종
의 감별이 임상적 또는 병리조직학적으로 불가능한 경우가 있다. 양성 병변도 크기가 매우
커질 수 있으며 피막 침범이나 림프 소포 소실(effacement of Iymph follicles)과 같은 소견
을 보일 수 있기 때문이다. 학자에 따라서는 양성과 악성 림프계 종양의 감별이 전적으로
세포 형태학적 소견에 의하여 결정된다는 의견을 제시하고 있지만 저자들의 증례에서는 이
러한 세포형태학적 기준이 림프계 종양을 분류하고 진단하는데 있어서 도움을 주지 못하였
다. 본 증례는 좌측 안와에서 발생하여 안와주위의 연부조직 및 골로 침윤하고 있는 림프계
종괴로서 수술로 부분 절제하였다.
저자들이 경험한 안와 림프계 종양은 세포 형태학적 및 면역 조직화학적 소견상 악성 림
프종으로 진단할만한 근거가 있었지만 국포적 방사선 치료 결과 및 환자에 대한 임상적 추
적검사 결과로는 양성 경과를 보이는 비정형적 가성 림프종(orbital pseudolymphoma)이었기
에 드문 증례로 사료되어 보고하고자한다.

Lymphoid tumors of the orbit are rare, and sometimes it is not Possible either
clinically or histologically to differentiate between Iymphoid tumor and pseudolymphoma.
Some authors assert that the degree of cytologic differentiation appears to be the
single most important factor for determining the prognosis of patients with orbital
Lymphoid lesions.
However, the cytomorphologic basis is not so helpful to diagnose and classify our
case, which shows some discrepancy between pathological findings and clinical and
radiological findings.
At first we misdiagnosed our case as orbital malignant Iyphoma on the basis of
cytomorphology and immunohistochemical study But no response to local intensive
radiotherapy and the follow up study of the patient suggest orbital pseudolymphoma.
