중성백혈구 증다증을 동반한 염증성 위암종

Abstract
종양조직에 대한 숙주반응은 종양의 특성에 따라서 여러가지로 표현되지만 림프계 세포를
포함하는 염증세포의 침윤이나 섬유화가 가장 대표적이다. 드물게는 일부 종양에서 육아종
을 형성하거나 심한 호산구 침윤을 나타내기도 하지만, 중성구의 심한 침윤을 동반하는 경
우는 악성 섬유조직구종을 제외하고는 매우 드물어 극히 일부의 악성종양에서만 산발적으로
보고되어 있을 분이다. 또한 이러한 중성구 반응은 비단 종양 조직에만 국한되어 있지 않고
말초혈액내의 비염증성 백혈구 증다증을 동반하며 종괴를 적출한 후 소실되기도 한다. 위암
종에 수반된 중성 백혈구 증다증 증례는 외국에서 Obara등의 보고가 있지만 우리나라에서
는 위암종의 발생빈도가 아주 높음에도 불구하고 1예도 보고되어 있지 않다.

A 36-year-old man received a subtotal gastrectomy for Borrmann type 2 advanced
carcinoma in the antrum of the Posterior wall. Preoperatively, peripheral white blood cell
count was 16,400/mn3 with 80% of neutrophils. Microscopically, the tumor
was of a poorly differentiated adenocarcinoma with intense neutrophilic infiltration which
was totally confined within the nooplastic Portion and in the metastatic focus in the
regional Iymph nodes. Repeated postoperative white blood cell counts dropped down to
the normal level. Neither infectious nor systemic disease with regard to a possible
source of leukocytosis was associated. To the best of our knowledge, this is the first
case of gastric carcinoma with massive neutrophilic stromal reaction(inflammatory
carcinoma) and leukocytosis in the literature in the korea. (Korean J Patrol 1993;27: 19
