Abstract
심장점액종은 심장에서 발생하는 종양중 그 빈도가 가장 높고(74.5%), 좌심방(50%)과 여
성에서 호발한다. 점액종의 여러형태의 종양세포는 난원와(fossa ovalis) 부위의 다능성 간엽
세포(pluripotenital mesenchymal cells)에서 기원한 것으로 생각하고 있고, 점액성 기질내에
는 다양한 양의 세망섬유, 교원섬유, 탄력섬유와 평활근육세포가 포함되어 있다. 염전
(torsion) 혹은 출혈등의 내외부적 자극에 의해 기질내 섬유조직들의 변성이 유발될 수 있는
데, 변성된 섬유조직에 출혈등에 의해 유출된 철이온이나 칼슘이온이 침착하게 되면 비장에
서 볼 수 있는 Gamna-Gandy 결절과 유사한 육아종성 병변을 보일 수 있다. 그러나 비교적
잘 형성된 육아종과 거대세포의 출현등에 대한보고는 매우 드물다. 색전증은 점액종의 임상
소견 중 승모판증(mitralism) 다음으로 흔한 임상양상으로 뇌동맥, 관상동맥과 폐세동맥이
호발부위이다. 저자들은 38세 여자환자에서 폐경색을 동반하고 특별한 원인균이나 혹은 전
신적 질환의 이차적인 영향없이 이물질형 거대세포를 동반한 비교적 잘 형성된 육아종성 병
변을 보이는 우심방점액종 1예를 경험하고서 그 예가 희귀하며 국내에서 보고된 바가 없어
보고하는 바이다.

Cardiac myxoma is the most common primary tumor of the heart. The tumor contains
a variety of cell types that are thought to arise from a focus of Primitive pluripotential
mesenchymal cells in the area of the fossa ovalis. Throughout the myxoid stroma, there
are variable amounts of reticular fibers, collagen, elastic fibers and smooth muscle cells.
A 38-year-old female had right atrial myxoma with multiple pulmonary infarcts. In this
case, we experienced an unusual degenerative change in the tumor of granulomatous
lesion consisting of hemosiderin pigments, foreign body giant cells and peculiar, spheroid,
semilunar or bamboo-shaped degenerated elastic fibers. Microscopically it resembles
Gamna-Gandy nodule seen in the spleen of chronic passive congestion. (Korean J Pathol
