Abstract
다형성 황색성상세포종(pleomophic xanthoastrocytoma, 이하 PXA로 칭함)은 1973년
Kepes등이 어린 연령층에 발생한 원발성 뇌종양 3예를 보고하면서, 대뇌피질의 표면부위와
수막사이에서 잘 발생하고, 세포질내에 풍부한 지방질 또는 호산성물질을 함유하는 원형, 방
추형 그리고 거대 세포들이 망상섬유에 의해 둘러싸여 있는 종양을 meningeal fibrous
xanthoma라고 부르면서 그 현미경학적 소견이 처음으로 기술되었다. 그후, Kepes등은 위와
같은 현미경적 소견을 보이면서 다른 종양으로 진단되었던 12예를 대상으로 면역조직화학적
염색과 전자현미경 검사를 실시하여, 이 종양세포들이 Glial Fibrillary Acidic
Protein(GFAP)에 양성을 보이고 세포질 주변에 기저판을 형성하면서 공통적으로 독특한 임
상양상을 보이기 때문에 이 종양을 성상세포종의 특수한 형태인 FXA로 분류하였고, 종양세
포들이 심한 다형성을 보이기 때문에 악성 종양과의 감별을 요하지만 후자에 비해서 좋은
예후를 보인다고 발포하였다. 외국문헌에는 약 40예가 발표되었고 국내문헌에는 불과 수례
만이 보고되었던 원발성 뇌종양이기에 저자들은 최근에 경험한 1예의 PXA에 대해 임상적
및 병리학적인 특징, 면역조직화학적 염색과 전자현미경적 소견을 관찰하고 문헌 고찰과 함
께 보고하고자 한다.

Pleomorphic xanthoastrocytoma is histologically characterized by marked cellular
pleomorphism of lipid-laden neoplastic astrocytes and bizarre giant cells showing mitotic
figures and high cellularity. Inspite of its ominous-looking microscopic features, however,
the prognosis is usually favorable. This tumor develops mainly in the supratentorial area
of young people and frequently involves the leptomeninges.
We experienced a case of pleomorphic xanthoastrocytoma in 18 year-old-male. In
addition to the cellular pleomophism, the prominent reticulin fibers surround the
individual tumor cells or he tumor cell nests. Immunohistochemical staining and electron
microscopy revealed glial fibrillary acidic protein(GFAP) expression and pericytoplasmic
basal lamina in the tumor cells. Korean J Patrol 1993; 27: 666∼669)
