Abstract
소아에서 발생하는 소화기계 악성 종양은 매우 드물고 이는 전체 소아 종양의 5% 미만을
차지한다. 그 중에서도 위의 악성 종양은 더욱 드물고 대부분이 악성 림프종이나 연부 육종
이며 상피세포암은 5% 미만이다. 소아 위의 상피세포암종은 조직학적으로 점액성 선암이
가장 흔하다고 보고되어 있다. 1939년 Phillips에 의해 10일된 유아에서 발생한 위의 상피세
포암이 최초로 보고된 이래 1976년에 Siegel등이 20개월된 소아에서 발생한 위의 점액성 선
암을 보고하였고 1988년에는 花田등에 의해 14세 남아의 위의 인환세포암이 보고된 바 있
다. 그러나 국내에서는 이 연령의 증례 보고가 없으므로 저자들이 경험한 1예를 보고하고자
한다.

Cancer of the alimentary tract in children is an extremely rare occurence, and it
accounts for no more than 5% of all pediatric neoplasms. Malignant neoplasms of the
stomach are particularly uncommon in childhood. The majority of such lesions are
malignant lymphomas or soft tissue sarcomas, with less than 5% identified as
carcinomas.
We report a case of signet ring cell carcinoma of stomach in 15-year-old girl. This
patient had a clinical manifestation of cough with sputum, fever, abdominal distension
due to ascites, hepatomegaly, and cervical lymphadenopathy for 40 days. There was no
known gastrointestinal symptoms. Metastatic signet ring cell carcinoma was suspected
by aspiration biopsy cytology of cervical lymph node, and confirmed later by excisional
biopsy. Endoscopy confirmed a small mucosal erosion in the anterior wall of the antrum
and histopathology confirmed the diagnosis of signet ring cell carcinoma.
