Abstract
위장관은 소화기능외에 혈장 단백의 대사와 분해과정에서 중요한 역할을 하며, 이러한 혈
장단백 대사와 분해에는 특히 소장의 역할이 크다. 장관을 통하여 소실되는 혈장단백의 양
을 정확히 알기는 어려우나 정상적으로 매일 혈장 알부민의 약 10∼20%가 장관의 내강으로
분비된다고 알려져 있다. 단백상실 장질환은 장관을 통하여 혈장 단백이 과다하게 소실되는
것으로서 그 원인은 위장관에 발생하는 여러 형태의 염증이나 종양 등 매우 다양하며, 이차
적으로 전신 부종, 복수늑막삼출, 림프구감소등을 초래한다. 장림프관 확장증은 림프관계의
선천성 기형으로서 단백 상실을 일으키는 원인의 하나이다. 장림프관 확장증 환자에서 단백
상실에 따른 임상증세는 주로 소아연령에 나타나지만 모든 환자에서 이러한 증상이 나타나
는 것은 아니기 때문에 그 정화한 빈도를 말하기는 쉽지 않다. 우리 나라에서는 1989년 1예
가 보고되어 있을 뿐이다.
저자들은 알부민투여에 반응하지 않는 심한 저단백증을 보인 3세 남아의 소장생검에서 장
림프관 확장의 소견을 관찰하였기에 그 발생기전 및 특성에 관한 문헌고찰과 함께 보고한다

This is a case report of idiopathic intestinal lymphangiectasia occurring in a 3 year
old boy. Idiopathic intestinal lymphangiectasia is believed to be a part of the generalized
congenital disorder of lymphatic system. The present case revealed markedly ectatic
lymphatics in the lamina propria of duodenal villi with lymphorrhagia. Clinical features
were typical of the protein losing enteropathy ; hypoalbuminemia, hypocalcemia, chylous
ascites, edema of the lower extremities and mild lymphocytopenia. This case indicates
that serious protein loss may ensue, even in cases of intestinal lymphangiectasia without
abnormalities of extraintestinal lymphatics.
