Abstract
안와내 수막종은 1816년 Scapa에 의해 처음으로 보고되었는데, 원발성은 매우 드물고 대
부분 두개강내에서 발생하여 안와내로 침윤된 속발성이며, 그 기원 부위를 알 수 없는 경우
도 있다. 원발성인 경우 종괴의 성장은 비교적 느리며 원격 전이는 거의 하지 않으나 경막
외로 펴져 나가거나 때로 안구내로 자라 들어와 공막, 맥락막, 시신경 유두 등에 병변을 일
으키기도 한다. 시신경초 수막종은 주로 중년 여자에서 발생하지만, 다른 부위의 수막종에
비하여 20세 이하에서도 발생 빈도가 높으며 이 경우 임상적으로 질병의 진행이 빠르고 재
발율도 높다고 하였다. 임상 중상은 안구 돌출과 시력 상실이 대부분이고, 다발성 섬유종에
동반되어 나타나는 경우도 있다. 국내 문헌에는 류 등이 보고한 1예를 포함하여 4예가 보고
되어 있을 뿐이다. 저자들은 최근 44세 여자 환자의 왼쪽 시신경에서 발생된 수막종 1예를
경험하고 그 예가 드물어 추가하여 보고하고자 한다.

Optic nerve sheath meningioma axises from the arachnoid cap cell of optic nerve
sheath and Comprises most of primary orbital meningioma. We experienced a rare case
of optic nerve sheath meningioma originating in the left orbit.
A 44-year-old woman had suffered from visual disturbance in the left eye fort 3
years and had only light perceptibility for the recent 6 months. The Tight eye was
normal. Brain CT scan and MRI revealed a 2×0.8 ㎝ sifted fusiform solid mass in the
left retrobulbar area. Under the impression of optic nerve tumor, excision of the mass
including a small segment of the optic nerve was performed. The tumor was a
yellowish firm, relatively well dermarcated mass that encircled the optic nerve without
infiltrating it. Microscopically it was a typical meningioma, meningothelial type. The
recovery of visual acuity was impossible due to resection of optic nerve but there was
no evidence of recurrence 책 11 months.
