Abstract
기무라병은 주로 동양인에 잘 생기는 만성 염증성 병변으로, 현재 원인이 잘 알려져 있지
않으나, 임상 및 병리학적으로 고유한 특징을 나타내는 질환이다. 조직학적으로 림프구와 호
산구의 침윤과 혈관 증식을 특징으로 하며, 최근까지도 여러 서양 문헌에서 호산구증을 동
반한 혈관 림프구성 증식(angiolymphoid hyperplasia with eosinophilia)과 동일한 범주의 질
환으로 간주되기도 하였다. 그러나 최근의 많은 논문들에서 이들이 서로 다른 범주의 질환
임을 증명하고있고 그 원인도 다른 것으로 보고하고 있다. 이 질환은 예후가 매우 좋은 질
환으로 보존적인 치료로도 완치가 가능하므로, 이 질환의 특징에 대해서 충분히 이해하고
정확히 진단함으로써 불필요한 광범위 수술을 피할 수 있다. 기무라병은 두경부의 피하 연
부 조직에 호발하나 림프절 종대로 나타날 수 있어 반응성 림프절 종대의 중요한 감별 진단
이 된다. 한편 이들 환자에서 신 질환이 잘 동반된다고 알려지고 있다. 이에 저자는 신 증후
군의 과거력이 있는 환자에서 발생한 기무라병 림프선염 1예를 경험하고, 이 질환에 대한
이해를 돕고자 이를 보고하고자 한다.

Kimura's disease is an important category of reactive Iymphadenopathy especially
among Asians. It is a chronic inflammatory disorder of unknown etiology and is
characterized by angiolymphoid proliferation and eosinophilia.
The patient was a 17-rear-old man with a 3-year-history of submandibular
Iymphadenopathy who was referred to Kyung Hee University Hospital for evaluation of
enlarging mass. He had a history of nephrotic syndrome for 7∼8 years since 1984.
Laboratory findings were not specific except for peripheral blood eosinophilia. The
submandibular Iymph node showed florid germinal centers, increased postcapillary
venules in the paracortex, eosinophilic infiltration, and sclerosis. In this paper, a case of
Iymphadenopathy of Kimura's disease associated with nephrotic syndrome is reported.
