Abstract
세포거대 바이러스 감염은 ① 자궁내에 있을때, ②출생후 어머니로부터 받은 항체의 소실
후, ③ 15세 이후에 빈발할 수 있는데 15세 이후에 발생하는 경우 원발성 감염은 드물며 대
개 잠복기 감염이나 무증상 감염의 경우가 대부분이다. 때로 혈액종양과 망상내피계 종양을
포함한 악성종양, 신이식, 스테로이드와 같은 면역억제제를 사용한 경우, 후천성 면역결핍증
환자에서 치명적인 기회감염으로 발현될 수 있다. 위장관계의 병변은 전신감염의 한 부분으
로 나타나거나 혹은 국소감염으로 발생할 수 있는데 임상증상은 복통, 설사, 혈변, 장출혈
및 장천공등이며 심하면 사망에까지 이를 수 있다. 최근 외과병리학적으로 세포거대 바이러
스감염의 진단은 DNA 표지자를 이용한 in situ hybridization 방법이나 중합효소 연쇄반응
을 이용하여 바이러스 DNA를 증폭시켜 진단할 수 있다. 저자들은 in situ hybridization 방
법으로 확진된 Henoch-Schoenlein 자반병에 의해 유발된 소장의 과민성 혈관염에 병발한
세포거대 바이러스 감염 1예를 보고하고자 한다.

Gastrointestinal cytomegalovirus(CMV) infection in adults is observed as a part of a
generalized or localized infection in patients who are immunocompromized.
We report a case of CMV infection of the small intestine. The patient is a 34 yeah-old
woman who has complained of palpable purpura in the lower extremities and buttocks,
and arthralgia of large joints. The skin biopsy showed fibrinoid necrosis and neutrophils
with leukocytoclasis, which findings are compatible with hypersensitiyity angiitis. The
patient received steroid and cyclophosphamide. During the follow-up period, generalized
edema and bloody stool were detected. Resected specimen of small bowel has multiple
aphthous ulcer. Microscopically, cytomegalic cells are observed along the endothelial cells
and mesenchymal cells. In situ hybridization using DNA probes against CMV revealed
positive staining in the cytomegalic inclusions in vascular endothelial and mesenchymal
