Abstract
요 약
본 연구에서는 종양절제술로 제거된 54예의 간세포암에서 극초단파 처치과정을 포함한 면
역조직화학적인 염색법으로 p53 단백발현을 조사하여 병리조직학적 소견과 비교분석하였다.
간세포암 54예중 25예(46%)에서 양성이었고, 주변에 동반된 간경변증 30예, 간선종 1예 및
선종성 과증식 2예에서는 음성이었다. 300 ng/㎖ 이상의 알파태아단백 수치와 p53 단백발현
과는 관련성이 있었으나 B형 간염바이러스 양성과 p53 단백발현과는 관련성이 없었다.
WHO 분류, Edmondson-Stainer 등급 및 임의로 정한 핵등급에서는 높은 핵등급이 p53 단
백발현과 유의한 관련성이 있었으나 종양의 크기, 피막형성유무, 종양의 간문맥 침습, 종양
주위조직의 간경변증유무, Eggel 분류, 특수 세포형 및 세포분열 수 등은 p53 단백발현과
관련성이 없었다.
결론적으로 간세포암에 있어서 p53 단백발현은 나쁜 병리조직학적 예후인자와 유의한 관
련성이 있어 간세포암의 예후추정에 있어서 보조적인 수단이 될수 있다고 생각하였다.

This study was carried out to evaluate the aberrant expression of p53 protein using
immunohistochemical method in 54 surgically resected liver cell carcinomas and to
correlate it with clinical and pathological findings. Twenty five out of 54 cases(46%)
showed positive reaction in the nucleus of liver cell carcinoma and negative reaction in
associated 30 cases of cirrhosis, one case of adenoma and two cases of adenoinatous
hyperplasia. The p53 protein expression was associated with α-FP level(p<0.05), but
not associated with HBsAg positivity. It was significantly associated with WHO
classification, Edmondson-Steiner grade and nuclear grade(p<0.05), but not associated
with tumor size, capsule formation, portal vein invasion, cirrhosis in surrounding tissue,
Eggel classification, special cell type and mitosis.
In conclusion, our results suggest that the aberrant expression of p53 protein can be an
advisory factor, at least, for prognosis evaluation.
