Abstract
흉폐부의 악성 소세포 종양(malignant small cell tumor of thoracopulmonary region)은
1979년 Askin등에 의해 처음 소개된 매우 희귀한 종양으로, 흔히 Aski 종양이라고 불리기
도 한다. 이 종양은 흉벽과 폐의 연부 조직에서 발생하며 국소적 재발이 흔히 관찰된다. 거
의 모든 증례가 소아기와 청년기에 발생하며, 여자에 좀더 호발 하는 것으로 보고되어 있다.
조직 소견상으로 원형 또는 난원형의 소세포들이 판상, 군상 혹은 분엽상등으로 조밀하게
배열되어 있는 것이 특징으로 흉폐부 이외에 발생하는 Ewing 육종과 매우 유사한 양상을
보인다. 면역조직화학적검색에서 neuron specific enolase등의 신경성 표지자에 대부분 양성
반응을 보이며, 초미세구조적으로 신경분비과립을 관찰할 수 있다. 이 종양의 발생기원은 아
직 뚜렷이 정립되어 있지 않으나, 신경 외배엽성인 말초 신경상피종(peripheral
neuroblastoma)의한 특수한 형태라고 인정되고 있다. 이 종양은 현재까지 문헌상 약 60예가
보고되어 있다.
최근 저자들은 12세 여자 환자의 우측 흉벽에서 발생한 악성 소세포 종양 1예를 경험하였
기에 그 소견들을 정리하여 문헌고찰과 함께 보고하는 바이다.

Malignant small cell tumor(MSCT) of the thoracopulmonary region(Askin tumor) is
extremely rare and is seen predominantly in children and adolescents. This tumor
represents a distinct clinicopathologic entity of neuroectodermal origin, arising from the
soft tissues of the chest wall or peripheral lung. This tumor tends to recur locally, but
does not seem to disseminate widely.
The overall survival is poor.
Recently, we experienced a case of the MSCT of the thoracopulmonary region of a
12-year-old female. She was admitted because of a chest wall mass on radiographic
examination, and a complaint of intermittent chest pain. Grossly, the mass was
lobulated, round and had a solid appearance with focal necrosis and hemorrhage on the
cut surface. Histologically, small round to oval cells were arranged in compact sheets,
nests and lobular patterns with intervening fibrovascular stroma. Ultrastructurally, the
presence of loose-fitting membrane-bound neurosecretory granules was noted.
