Abstract
육아종성 균상 식육종은 균상 식육종의 드문 조직학적 변형으로1, 1970년 Ackerman과
Flaxman이 처음으로 명명하였으며, 임상적으로 광범위한 궤양성 소절로 나타나서 위축성
반흔이나 다형피부위축증의 반을 남기면서 저절로 치유되며, 조직학적으로 육아종성 악성
림프종의 양상을 보이는 것으로 설명하였다. 균상 식육종에서 육아종이 나타나는 경우는 아
주 드물지만, 이들의 존재가 양호한 예후와 연관이 있다는 보고들이 있다. 또한 육아종성 균
상 식육종은 비종양성 피부염이나 사르코이도시스로 오진되는 경우가 종종 있으며, 그외 육
아종을 보이는 다른 림프종과의 감별이 중요하다.
이처럼 육아종성 균상 식육종은 임상-병리학적 관심의 대상이 될 뿐 아니라, 전 세계적으
로도 아주 드물며, 국내 문헌에서는 보고된 예를 접할 수가 없었다.
이에 저자들은 34세 남자의 좌측 뺨에 생긴 육아종성 균상 식육종 1예를 경험하고, 문헌고
찰과 함께 보고한다.

Granulomatous mycosis fungoides is an extremely rare and unusual histologic variant
of mycosis fungoides. This form is clinically characterized by spontaneous resolution of
ulcerated nodular lesions into poikiloderma. Histologically, a strong granulomatous
component can obscure the underlying cutaneous lymphoma, which is frequently
mistaken for non-neoplastic dermatitides or cutaneous sarcoidosis.
We _report a case of granulomatous mycosis fungoides occurring on the left cheek of
34-year-old man confirmed histologically with an aid of immuilohistochemistry and
clinical course (immediate response to PROMACE-CYTOBAM chemotherapy), with
emphasis on differential diagnosis, along with a review of literature. This is the first
documented report in the Korean literature.
