Abstract
고분화 흉선암은 주로 종격동에 발생하는 원발성 상피암으로 1989년 Kirchner등이 처음으
로 피질 흉선종에서 분리하여 보고하였다. 그후 1992년 Kirchner, Pescarmona등이 각각 15
예 및 26예의 고분화 흉선암을 보고하면서 그 임상적 및 조직학적 특징을 기술하였다. 그후
1993년 Leticia-Quintanilla등이 32예, 1994년 Chin-Chen Pan등이 12예의 고분화 흉선암을
추가로 보고하였다. 이들이 보고한 예의 대부분은 이전에는 피질 흉선종으로 분류되던 것으
로, 조직학적 소견상 상피세포는 피질 흉선종과 유사하나, 세포의 이형성이 있고 유사분열이
존재하며 피질 흉선종보다 더 빈번하게 주변 조직으로 침윤하는 것 등이 특징이다
Leticia-Quintanilla등은 32예의 고분화 흉선암 중 그 이전에는 인지되지 않았던 아형으로 방
추세포형 고분화 흉선암 6예를 보고하였다. 방추세포형 고분화 흉선암은 방추세포로 구성되
기 때문에 이전에는 수질 흉선종 또는 방추세포형 흉선종으로도 분류되었다. 그리나 최근에
는 이득 방추세포의 이형성 및 유사분열의 존재와 부분적으로 편평상피로 분화를 보이는 점
을 들어 이들을 방추세포형 고분화 흉선암으로 따로 분류하고 있다. 최근 저자들은 66세 여
자의 전격동에서 발생한 매우 드문 방추세포형 고분화 흉선암 1예를 경험하였기에 흉선종의
분류에 대한 고찰과 함께 보고하는 바이다.

Well differentiated thymic carcinoma(WDTC) was recently separated from cortical
thymoma. It is characterized by a predominance of epithelial cells with usually low
mitotic rate, an epidermoid differentiation with slight to moderate cytologic atypia and
lobular growth patters. In recent reports, an uncommon spindle cell variant of WDTC,
which is composed of spindle shaped epithelial cells, has been discribed. We investigated
an unusual case of WDTC consisted of purely spindle shaped epithelial cells in a
66-year-old female. Radiologically, the well demarcated mass was located in the anterior
mediastinum with focal invasion into the surrounding left upper lung. The tumor, 10×8
×5 ㎝, was encapsulated with thin fibrous tissue and showed a pale yellow solid and
lobulated cut surface. Microscopically, it consisted of solid sheets of purely spindle
shaped epithelial cells with mild atypism, a low mitotic rate and focal epidermoid
