Abstract
모 기원의 피부 부속기 종양중 모의 초기 발육단계에서 기원하는 모세포의 미성숙 종양은
드물게 관찰되늘 종양으로서 1962년 Headington과 French등이 처음으로 모(毛)기원 종양을
분류하였고, 그후 1970년 Headingtonl은 2예의 모세포 종양을 보고하면서 이를 모모세포종
(trichoblastoma), 모모세포성 섬유종(trifoblastic fibroma), 모분화성 모모세포종(trichogesic
trichoblastoma), 모분화성 점액종(trichogenic myxoma)등 4가지로 분류하였으며 이의 분류
에 따라 여러 저자들3.4.5의 보고가 있었다 그러나 그후 Ackerman은 이들이 조직학적으로
유사한 종양들이라고 생각하여 모두 모모세포종(trichoblastoma)으로 부르고 있어서 아직도
용어가 정립되고 있지 않다. 또한 Headington이 분류한 어느 군에도 속하지 않으면서, 조직
학적으로 다양한 양상을 보여주는 예들에 대해 1992년 Sau등은 모세포기원 종양 14예를 보
고하면서 모배아종(trichogerminoma)이라는 용어를 최초로 제시하였다.
저자들은 51세 남자의 등에 발생한 모세포 기원피부 부속기 종양 중 조직학적 소견이 Sau
등이 보고한 모배아종과 일치하는 1예를 경험하였기에 문헌고찰과 함께 이를 보고하는 바이
다.

Though trichogenic tumors were classified as trichoblastoma, trichoblastic fibroma,
trichogenic trichoblastoma and trichogenic myxoma by Headington(1970), their true
classification depends upon the epithelial and mesodermal component as well as evidence
of their induction. Because of the rarity of hair germ cell tumors their classification is
still controversial. In this report, we describe a case of trichogerminoma which is not
included in the above classification. The trichogerminoma was first described by Sau et
at. in 1992 and characterized by its morphologic pattern of germinal centers and
Iymphoid follicle-like structures in the nests of trichoblasts. Herein we reporte a tumor
which arose on the skin on the back of a 51-year-old man and presented as a sharply
circumscribed mass(4.5×2.0×1.5 ㎝) involving both the dermal and subcutaneous tissues
without any epidermal connection. The tumor had many germinal center-like structures
in the basaloid trichoblasts. Lobular cell nests were separated by variable amounts of
stroma, but no horn cyst were noted. The germinal center-like cells showed early
differentiation of hairs, resembling early hair bulbs. Trichogerminoma is considered to be
a type of tumor located between trichoblastoma and trichoblastie fibroma.
