Abstract
혈관근섬유모세포종(Angiomyofibroblastoma)은 1992년 Fletcher등1이 침습성 혈관점액종
(Aggressive angiomycoma), 점액양 혈관지방종(Myxoid hemangiolipoma) 또는 점액양 지방
육종(Myxoid liposarcoma) 등으로 보고된 10예의 외음부 연부조직 종괴 병변을 병리조직학
적, 면역조직화학적 그리고 전자현미경적 소견에 따라 재분류하면서 처음으로 제안된 새로
운 종양 질환이다. 이 질환은 임상적으로 30∼40대 여성의 외음부의 표층에서 경계가 좋은
작은 결절로 나타나고 절제 후 재발을 하지 않는다는 점에서 침습성 혈관점액종과 감별을
요하는 질환이다. Flecher등이 10예를 처음 보고한 이래 전 세계적으로 3∼4예가 더 보고되
었지만 국내에서는 대한병리학회 월례 집담회에서 1예(KSP 901)가 토의된 적이 있으나 문
헌으로 보고된 바는 없다. 최근 저자들은 외음부에 아주 드물게 발생하는 혈관근섬유모세포
종 1예를 경험하였기에 이 질환의 조직학적 특성과 기원설에 대한 간단한 문헌고찰과 함께
증례를 보고하고자 한다.

Angiomyofibroblastoma is a distinctive, hitherto uncharacterized, benign soft tissue
tumor of the vulva with histology similar to an aggressive pelvic angiomyxoma. It can
be distinguished from an aggressive angiomyxoma by its circumscribed borders, higher
cellularity, more numerous blood vessels, the frequent presence of plump stromal cells,
minimal stromal mucin, and rarity of erythrocyte extravasation.
We experienced a case of angiomyofibroblastoma of the vulva occurring in a
45-year-old woman. The lesion was a well-defined but not encapsulated mass, 4.5×4.2
㎝. Histologically the mass was characterized by alternating hypercellular and
hypocellular edematous zones in which abundant blood vessels were irregularly
distributed. Immunohistochemically, the spindled, plump spindled, and oval stromal cells
were reactive for vimentin and destain, but not for cytokeratin, or S-100 protein.
