소아에 발생한 흉막폐 모세포종

Abstract
전형적인 폐모세포종은 암육종의 한 유형으로 원시적인 관상구조, 미성숙 모체 및 방추세
포 기질로 구성되고 대부분 성인에서 발생한다. 이에 비해 소아에 발생하는 대부분의 폐모
세포종은 배성 횡문근 육종이나 모체성 종양과 육종이 혼합된 간엽성분으로 구성되고 일부
는 발생 낭종으로 오인되는 말초 다낭성 병변으로 나타난다. 소아에 발생하는 이러한 폐 혹
은 폐외 종양을 Spencer가 기술한 성인의 폐모세포종과는 구별하여 흉막폐 모세포종이라
제안하였고 그 중 특히 제 1형은 예후가 양호하므로 감별진단이 필요할 것으로 생각된다.
저자들은 최근 긴장성 기흉으로 내원한 27개월된 남아에서 특징적인 제 1형의 흉막폐 모세
포종을 경험하여 면역조직화학소견과 함께 보고하는 바이다.

Classic pulmonary blastoma is a variant of carcinosarcoma which is seen almost
exclusively in adults. By contrast, most cases of pulmonary blastoma in children have
been described as having an exclusive mesenchymal composition, which was proposed
as pleuropulmonary blastoma.
Recently we experienced a case of pleuropulmonary blastoma, type 1. This
27-month-old male baby was transferred to our hospital due to the left tension
pneumothorax Chest CT revealed a subpleural pulmonary cystic lesion on the left upper
lung and an open cystectomy was performed. Histologically the lesion was composed of
variable-sized cystic structures lined with a single layer of respiratory-type epithelium.
the underlying stroma was composed of sheets of small, round to oval, primitive tumor
cells. Some of them had eccentric, eosinophilic cytoplasm, suggestive of
rhabdomyoblastic differentiation. These rhabdomyoblastic cells were fuchsinophilic and
positive with desmin and vimentin on immunohistochemistry.
