Abstract
타액선의 양성 림프 상피낭(benign lymphoepithelial cyst)은 드문 질환으로서 경부의 양
성낭의 2%를 차지하며 이하선 절제조직에서는 1% 미만으로 알려져 있다. 일관된 역학 자
료는 구할 수 없지만 이 질환은 빈도에 있어서 성별에 따른 유의한 차이는 보이지 않으며
대개 이하선 부위에 일측성의 무통성 종괴로 나타나는 경우가 대부분이다. 그 조직학적 기
원에 대하여 많은 논쟁이 있었는데 1958년 Bernier와 Bhaskar는 타액선의 림프 상피 병변
186예를 대상으로 한 연구에서 양성 림프 상피낭을 타액선내 림프절에 생긴 단발성 혹은 다
발성 낭으로 정의하였고 림프절내에 봉입된 타액선 조직이 낭성 변화를 일으켜 발생한 것으
로 추정하였다. 그 외 다른 학자들은 아가미궁(bronchial arch)을 양성 림프 상피낭의 기원
으로 여기고 보고하고 있기도 하여 현재 두 가지 가설이 모두 받아들여지고 있다. 임상적으
로 이병변은 재발하거나 전이하지 않는 양성병변이어서 외과적 절제만으로 치유되므로 병리
적으로 다른 낭성 타액성 종양과 감별 진단하는 것이 중요하다 저자들은 신경섬유종증 환자
에서 병발한 양성 림프 상피낭 1예를 경험하여 보고하는 바이다.

An intraparotid benign 1ymphoepithelial cyst is a rare disease characterized by
unilateral painless swelling of parotid region. The histogenesis is controversial. Surgical
excision is recommended for diagnosis and curative treatment. We present a case of
benign lymphoepithelial cyst arising in a patient with neurofibromatosis. A46-year-old
woman presented with a slowly growing multilocular cystic mass in the left cheek. The
cystic mass measured 4 ㎝ in maximal outer diameter and the cystic wall was thick
and yellowish pale to gray, soft with well circumscribed margin. Microscopically, the
multilocular cyst was lined by stratified squamous epithelium for the most part and
underlying lymphoid tissue aggregates with follicles and sharply demarcated from
adjacent salivary parenchyma which is of normal appearance and without lymphoid
aggregates. Since this lesion is absolutely benign, it is important to separate this benign
cyst from cystic salivary gland tumors. (Korean J Pathol 1996; 30: 551 ∼553)
