Abstract
부신에 발생하는 낭종은 아주 드물어서 우리나라의 경우 1967년 염의 위낭종 1예, 1982년
강등의 상피성 낭종 1예가 보고되었으며, 대부분 크기가 작고, 임상증상을 동반하지 않기 때
문에 부검시 우연히 발견되는 경우가 일반적이다. 그러나, 일부의 경우 임상증상을 초래할
수 있는데, 이 경우 이 질환의 희귀성에 의해 진단이 어려워져 오진을 내리는 경우가 흔히
발생된다. 또한 부신에 발생된 낭성변화를 보이는 양성 및 악성 종양과의 감별이 요구되며,
주변장기 특히 간장, 비장, 신장 및 췌장의 낭성병변과도 감별이 필요하다. 저자들은 정기
신체검사도중 초음파 검사에 의해 우연히 발견되어, 그 당시 췌장의 가성낭종으로 오인하였
던 44세 남자의 좌측부신에 발생된 내피성 낭종 1예를 경험하였기에 문헌고찰과 함께 보고
하는 바이다.

Adrenal cysts are rare lesion that usually present themselves as an incidental finding
during surgery, or at the time of autopsy. The cysts are usually small, seldom
exceeding loom in diameter, and are generally asymptomatic. However, they present a
difficult problem in differentiation between benign and malignant lesions of the adrenal
gland. In the differential diagnosis, other cystic lesions of the upper abdomen must also
be considered, including hepatic, splenic, renal and pancreatic cysts. Herein we report a
case of endothelial cyst of lymphangiomatous type of the adrenal gland which was
detected in a 44-year-old male patient during a routine health examination by
ultrasonography as a pancreatic pseudocyst. Gross examination revealed multiple
separate but continuous cysts, measuring 10.6×8×7㎝ in dimension. Within the wall,
compressed adrenal cortex was noted. Microscopically, fibrous wall containing
hypertrophied smooth muscle lined by endothelial cells was also noted. We reviewed
literatures of the adrenal cyst and report a case.
