Abstract
선근종(adenomyoms)은 자궁내막에서 발생하는 용종성 종양의 약 1%를 차지하는 양성종
양으로서 그중 내막선의 이형성 변화를 보이는 이형성 용종양선근종(atypical polypoid
adenomyoma, APA)은 1981년 Mazur에 의해 처음 5예가 보고된 이래 지금까지 전세계적으
로 약 39예가 보고되어 있다. APA는 조직학적으로 평활근으로 이루어진 간질내에 이형성을
보이는 증식성 내막선들로 구성되어 있어, 특히 소파수술 검체에서는 자궁근층을 침범한 자
궁내막 선암종으로 오인되기도 한다. 내막선과 간질에서 이형성과 함께 소수의 세포분열이
관찰되고, 소파수술후 재발하는 경우도 있지만, 문헌보고에 의하면 추적조사 결과 양성종양
으로 알려져 있다. 저자들은 2예의 APA를 경험하고 아직 국내 문헌보고가 없어 smooth
muscle-actin, desmin에 대한 면역조직화학적 염색소견과 함께 보고하는 바이다.

Atypical polypoid adenomyoma(APA) is a rare benign polypoid tumor arising in the
uterine endometrium which was first designated by Mazur in 1981. Microscopic
examination reveals architecturally and cytologically atypical endometrial glands
separated by intersecting fascicles of smooth muscle cells. The tumor can be
misdiagnosed as endometrial adenocarcinoma with myometrial invasion, especially on
microscopic examination of a curettage specimen. However a granulation tissue-like
stromal response is absent and the smooth muscle in APA is more cellular than that of
normal myometrium.
We are reporting 2 cases of APA of the uterine endometriunl which are 1.5 cm and 1
7 cm in size, respectively, in a 30 and a 22 year-old women. This is the first report in
Korean literature using the immunohistochemical staining for smooth muscle actin and
desmin.
