Abstract
에크린 한관의 상피는 다양한 자극에 의해서 드물게 화생을 일으킬 수 있으며, 두가지 주
요 유형, 즉 점액상 이형(mucinous variant)과 편평세포 이형이 알려져 있다. 편평세포 이형
은 입방형의 에크린 관상피가 성숙한 편평상피로 화생을 일으키며, 점액상 이형은 에크린
분비상피가 원주 잔모양(goblet cell) 세포로 화생을 일으킨다. 이 중 에크린 한관 편평세포
화생(eccrine squamous syringometaplasia, ESS)은 1974년 Freeman이 압박성 괴사용해
(pressure necrolysis)와 울혈성 궤양에서와 같은 비특이적인 조건에서 발생한 예를 처음으
로 기술한 이래 다양한 선행 질환이나 조건들과 연관하여 발생하였던 예들이 보고되었다.
이 질환의 중요성은 흔히 병리조직학적으로 편평세포암종으로 오진될 수 있다는 점이다. 저
자들은 최근 재생불량성 빈혈로 내원하여 치료받던 15세 남자에서 진피 하부에서 에크린 선
주변의 지방 조직에서 급성 염증 소견과 동반하여 발생한 에크린 편평세포화생 1예를 경험
하여 보고하는 바이다.

An eccrine squamous syringometaplasia (ESS) is defined as a mature squamous
metaplasia of the eccrine ducts. The clinical and pathological features of an ESS are
presented. Syringometaplasia is a rare lesion mostly occuring in the extremities, and as
far as we know, no report on the ESS has been published in Korean literature. We
experienced a case of an ESS occured in a 15 year-old male, who had a tender
erythematous plaque in the right flee. The histologic examination revealed some
scattered nests of metaplastic squamous epithelium in the deep dermis, associated with
acute nonspecific panniculitis. The importance of the ESS is that it histologically
simulates the well differentiated squamous cell carcinoma. The histopathologic findings
were discussed and a brief review of the literature was made.
