Abstract


"Lethal midline granuloma is a disorder in the granulomatosis-angiitis disease spectrum and characterized by necrotizing granulomatous lesions in the upper respiratory tract or in the lower respiratory tract or both, generalized focal necrotizing vasculitis involving both arteries and veins and granulomatous glomerulitis. Although the etiology is felt to be hypersensitivity confirmatory evidence is lacking. Adequate and repeated biopsies are necessary to make a diagnosis and to rule out a neoplastic process. In general, the treatment of lethal midline granuloma has not been successful but massive steroid therapy, chemotherapy such as immunosuppressive agents and radiotherapy, or combination of these were known to be beneficial to this disease. Most of patients die within a year after the onset. Herein the authors report a case of lethal midline granuloma metastasized to the prostate and perineum with the review of literatures."
