Abstract
서론
아밀로이드증은 균질의 호산성 유리질의 단백질이 침착되는 것으로 우리 몸 어디에서나
국소 또는 전신적으로 발생할 수 있는 질환으로서, 경우에 따라 생명에 심각한 영향을 미칠
수 있다. 아밀로이드증은 이미 발생된 병변의 감소 또는 소실이 거의 발생하지 않고, 축적이
계속 진행되기 때문에 병변 조직의 위축을 초래한다. 현재까지 비뇨기계의 아밀로이드증의
발생율 및 보고례의 정확한 정보는 없으나, 원발성 아밀로이드증은 흔하지 않다. 아밀로이드
증이 발생되는 비뇨기계장기로서는, 신장, 요관, 방광 및 전립선 등이 알려지고 있다. 방광의
원발성 아밀로이드증은 매우 드물게 발생하나 이 등이 발현 양상이 방광암과 매우 유사하여
반드시 방광암과 감별진단이 필요하나, 육안적으로는 쉽지 않으며, 치료방법과 치료에 대한
예후가 방광암과 매우 다르다.
본교실에서는, 타병원에서 체외충격파 쇄석술에 의한 요관결석 치료중 실패하여 본원으로
전원된 환자의 치료 중 우연히 발견된 방광 아밀로이드증을 치료하였기에 문헌 고찰과 함께
보고하는 바이다.

Primary bladder amyloidosis is a rare disease causing hematuria which is difficult to
be differenciated from bladder cancer at cystoscope. We report a case of primary bladder
amyloidosis who was diagnosed at other procedure for distal ureteral stone failed in
repeated ESWL disintegration. Transurethral resection of bladder mass and the
pathologic results revealed amyloidosis. The systemic studies for the detection of the
site of other amyloidosis were failed to get positive result. There were massive
hematuria after a few hours later from the transurethral resection of the bladder mass
and the bleeding was controlled with 1% alum bladder irrigation. The patient is followed
regularly for recurrent amyloidosis.
