외상성 완전요도협착 치료에 있어서 직시하 내요도절개술과 요도성형술의 비교

Abstract
결론
완전 요도협착에 대한 직시하 내요도절개술과 요도성형술의 성공률, 합병증 및 성공에 영
향을 미치는 인자를 분석한 결과 일차성공률에서는 1㎝ 미만의 경우 직시하 내요도절개술은
40%, 요도성형술은 90%였고, 1 ㎝ 이상의 경우 직시하 내요도절개술은 12.5%, 요도성형술
은 83.3%였다. 추가시술의 횟수에 상관없이 6개월 이상 요착이 재발되지 않고 배뇨상태가
양호하며 요류속도가 15㎖/sec 이상인 이차성공률에서는 직시하 내요도절개술의 경우 1 ㎝
이하에서는 1 ㎝이상에서는 37.5% 로서 1 ㎝이상에서 의의 있게 직시하 내요도절개술의 성
공률이 감소하였다. 따라서 완전 요도협착와 길이가 1 ㎝ 인 경우 잦은 협착의 재발로 여러
번의 내요도절개술이 필요하였으나 재발간격이 길어지고 합병증의 발생이 적고 실패하여도
요도성형술을 시행하는데 지장을 주지 않는 장점이 있는 직시하 내요도절개술이 일차적으로
고려해볼만한 방법으로 생각되며, 1 ㎝ 이상의 경우 결손부위 개통이 어렵고 개통에 성공하
더라도 협착이 계속 재발되는 경우가 많아 요도성형술이 적합한 것으로 사료된다.

Visual internal urethrotomy may be a reasonable initial procedure of complete and
incomplete urethral stricture before planning more extensive urethroplasty. We reviewed
our experience with 62 complete urethral strictures for 10 years. The outcomes of the
treatment of 28 patients who were managed by visual internal urethrotomy were
compared with those of 34 patients managed urethroplasty, The overall successful
results were 32.1% in visual internal urethrotomy and 85.3% in urethroplasty. When
stricture length was less than 1 ㎝, success rate was 40% in spite of several
recurrences in visual internal urethrotomy, comparable to the success rate of 12.5% in
case of stricture length more than 1 cm. We recommended that visual infernal
urethrotomy as an applicable initial method before urethroplasty, when stricture is less
than 1 ㎝ in cases of complete urethral stricture
