다발성 신석 및 농신증이 동반된 육종양신세포암 1례

Abstract
서론
육종양신세포암은 전제 신실질 종양의 약 1∼1.5%를 차지하는 비교적 드문 악성종양으로
육종양성분과 전형적인 신세포암의 상피세포성분이 혼합되어 발생한다. 육종양신세포암은
신세포암 중에서도 예후가 극히 불량하여 술후 대부분 1년 이내에 사망한다고 보고되고 있
다. 따라서 육종양 신세포암을 다른 신종양과 감별하는 매우 중요시되고 있다. 육종양신세포
암의 진단은 수술시 절제된 조직의 H-E 염색 및 조직 면역화학적염색에서 상피세포성분과
육종양성분 혼합되어 있는 것을 관찰함으로써 이루어진다.
최근 저자들은 고열 및 우측복부동통을 주소로 내원한 77세 여자 환자에서 다발성 신석,
신출혈 및 농신증이 동반된 육종양신세포암 1례를 치험하였기에 문헌고찰과 함께 보고한다.

Sarcomatoid renal cell carcinoma is uncommon and constitutes approximately 1.0% to
1.5% of all renal parenchymal tumors. Renal tumors that consist of epithelial and
sarcomatoid components were called Sarcomatoid renal cell carcinomas. Its distinction
from other morphological variants of renal cell carcinoma is significant, because it has
been shown to have a much poorer prognosis than the purely epithelial tumors. The
diagnosis of sarcomatid renal cell carcinoma is usually made when epithelial and
sarcomatoid components are mixed in nephrectomy specimen.
A case of sarcomatoid renal cell carcinoma is herein presented in a 77 year-old
women. she complained right flank pain and high fever for 3 days, radical nephrectomy
was performed and diagnosis was made by H-E stains and immunohistochemical stains
of nephrectomy specimen.
A brief review about sarcomatoid renal cell carcinoma is made.
