Abstract
서론
간질성 방광염은 요검사에서는 정상이나 만성적이고 지속적인 빈뇨, 급박뇨, 치골상부통.
배뇨통 등의 증상을 호소하며 방광경검사에서 특징적인 점막소견을 보이는 질환이다.
1887년 Skene가 처음으로 이 질환에 대하여 언급한 이후, 여러 사람들이 이 질환에 대한
연구를 하였으나 아직까지 정확한 원인 및 치료에 대하여 밝혀져 있지 않다 현재 원인은 여
러 가지 가설로 설명되고 있으며, 따라서 치료방법 또한 다양하게 시도되고 있다. 이 질환은
미국에서는 비교적 흔하게 보고되고 있으나, 국내에서는 최근에 이르러서야 드물게 연구 및
보고가 이루어지는 실정이다. 수압방광확대술은 방광점막, 신경조직, 및 배뇨근 등에 허혈성
변화와 손상을 주어 증상 완화효과가 나타난다고 하며 비교적 간단하고 반복 시행할 수 있
다는 것이 특징이다.
본 교실에서는 급박뇨, 빈뇨, 치골상부통, 및 배뇨통 등의 증상을 보인 환자에서 요류역학
검사, 방광경검사 등을 시행하여 National Institute of Arthritis, Diabetes, Digestive and
Kidney Disease(NIDDK)에서 제시한 exclusion criteria를 기준으로 간질성 방광염을 진단하
였다. 진단된 19명의 환자를 대상으로 단순 수압방광확대술을 실시하였으며 치료 후의 효과
를 분석하였다

Purpose : We reviewed 19 patients with interstitial cystitis from March 1992 to
August 1996. All patients were treated with simple hydrodistention of bladder, and we
followed the results of their effect.
Materials and Methods : Most of the patients were middle aged women(mean age:
50.4 years). On cystoscopy, glomerulations were noted in all patients. Major symptoms
were frequency(100%), dysuria(74%), urgency(74%) and suprapubic discomforts(47%).
Simple hydrodistention was used in all patients. Mean bladder capacity was
150m1(50-280) in 1 month after procedure compared with 114m1(50-190) before
procedure. Hematuria was noted during postoperative period in all patients.
Results : As the result of the postoperative urodynamic study, 5 out of 19
patients(26.3%) had no more symptoms, 9 patients(47.4%) was improved, but 5
patients(26.3%) showed no improvement. Sixteen of 19 patient were followed up
1.5-14(mean: 71) months, previous symptoms were recurred in 12 patients 3-4 months
after the treatment.
Conclusions : Even though the results show short-term effects, simple bladder
hydrodistention would be one of the effective treatment modalities of interstitial cystitis.
