Abstract
서론
방광이행세포암에서의 방광내 bacillus CalmetteGuerin(BCG)주입요법은 광범위한 임상적
시도에서 국소 면역치료로서 세포 독성물질의 방광내주입의 우수성이 입증되었다. BCG는
방광에서 염증반응을 유발하는데 이것은 국소 항종양성 반응과 연관되는 것으로 간주되고,
방광내 BCG주입요법의 효과는 BCG의 생존력과 관련되어 있다. Kelley등은 표재성 방광암
환자에서 방광내 BCG의 생존력이 유의하게 차이가 있으며 BCG 방광내주입요법 후 주입된
살아있는 BCG의 생존력이 높았던 환자에서 치료결과가 우수했다고 보고하였다. Bohle 등은
실험적으로 방광암에 대한 면역요법에서 BCG에 의해 활성화된 killer cell의 유도에 필요한
살아 있는 BCG의 효과적인 적정량을 정립한 바 있다. 이러한 사실은 방광내 BCG주입에
의한 면역요법에서 주입된 BCG의 생존력과 생균의 수는 임상적으로 의미가 있다는 사실을
말해 준다. BCG의 방광내주입시 요도의 손상을 줄이기 위해 일반적으로 윤활제를 사용하
며, 이에는 일반적으로 항균물질이 포함되어 있는데 이것이 BCG와 혼합되었을 때 BCG의
생존을 억제하여 방광내 에서 면역반옹을 저하시킬 수 있다. 이에 저자는 도뇨시 방광내로
유입될 수 있는 윤활제가 BCG의 생존력에 미치는 영향을 죠사하기 위해 BCG와 윤활제를
여러 조건에서 배양하여 그 결과를 비교하였다.

Purpose : Intravesical instillation of bacillus Calmette-Guerin(BCG) is an established
and effective therapy for the superficial bladder carcinoma. The viability of BCG is
crucial for the induction of a local immune response as well as effective therapy of
recurrent superfical bladder carcinoma. Lubricants are used to facilitate catheterization
during intravesical instillation of BCG. Moreover bacteriostatic components contained in
them have potential to reduce the viability of the BCG. To verify this assumption,
inhibitory effect of four commercially available lubricants on the BCG growth was
analyzed. Materials and Methods : Four different lubricants and their components were
co-incubated with Connaught strain BCG and the resultant growth of BCG was
assessed.
Results : Significant impairment of BCG viability with lubricants was noted.
Chlorhexidine digluconate which is the component of lubricant was considered as
responsible for this inhibition.
Conclusions : During intravesical BCG, lubricants might reduce the number of viable
BCG in clinical use. For this reason, during intravesical immunotherapy with BCG small
amounts of lubricants should be used for urethral catheterization and use of lubricant
which does not contain bacteriostatic agent should be considered. (Korean J Urol 1998;
