Abstract
서론
전림선은 사정과 배뇨를 조절하는 남성 생식기 관이므로 전립선에 병변이 생기면 배뇨증
세, 통증과 같은 신경통증세, 성기능에 관련한 증세 등의 전립선증상이 다양하게 발현할 수
가 있다. 이러한 증상을 호소하는 청장년의 환자들은 대부분 전립선염 혹은 전립선통으로
진단된다.
전립선염과 전립선통과의 진단기준이 단지 전립선액 검사에서 백혈구가 증가되었다는 소
견으로 분류하기 때문에 검사자의 술기와 전립선낭 개구부의 폐쇄 등의 요인으로 위음성으
로 나올 가능성이 많이 있으므로 정상 범위의 백혈구 소견을 보여도 전립선염이 있는 경우
도 있다. 따라서 최근에는 이를 통합하여 전립선 증후군이라 명명하고 있다.
이러한 환자들이 호소하는 증상은 매우 다양할 뿐만 아니라, 주증상을 치료하여 소실되면
다른 증상을 호소하고, 이를 또 치료하면 다시 또 다른 증상을 호소하는 경향이 있다 따라
서 치료하는 의사들은 환자가 호소하는 다양한 증상을 처음부터 파악하지 않고 이러한 형태
의 증상을 호소하는 환자들에게 정신병적인 문제를 제기함으로써 일부 환자들은 불쾌감을
받았다고 호소하고 있는 실정이다.
따라서 본 연구는 청장년 전립선 증후군 환자들을 대장으로 동통에 관련한 신경통증상 6
가지와 배뇨에 관련한 증상 11가지와 성에 관련한 증상 7가지 총 24가지증상에 관련한 내용
을 포함한 설문서를 이용하여 전립선증상의 다양한 발현 양상을 조사함으로써 이러한 환자
들을 진단하고 치료하는데 참고자료가 되게 하고자 한다. 아울러 전립선염군과 전립선통군
에서의 증상 발현을 비교 분석함으로써 전립선염의 특징적인 증상을 알아보고자 한다.

Purpose : Chronic prostatitis is characterized by various symptoms. To elucidate the
prevalence of such symptoms, we developed a questionnaire composed of six
pain-related, seven sex-related, and 11 voiding-related questions and evaluated chronic
prostatitis patients under 50 years of age.
Materials and Methods : From March 1995 to July 1996, a total of 500 patients
(prostatitis 303, prostatodynia with history of prostatitis 153, prostatodynia only 44)
underwent study. The mean age was 35 years and the average duration of infliction
was 3.5 months.
Results : Perineal, testicular, and penile pains were the most often complained types of
perceived pains. The most frequently complained voiding symptoms were frequency,
weak stream, residual urine sensation and dribbling which were seen in over 40% of the
patients. More than 50% of the patients responded as having some type of voiding
difficulty. For sexual symptoms, premature ejaculation , decreased libido and orgasm,
decreased potency, and pain associated with ejaculation were seen in 64%, 55%, 54%,
and 47% of the patients. When the prostatitis and prostatodynia patients were compared
to one another to find out any characteristic symptoms of prostatitis, there was no
difference in pain but statistically significantly higher response was observed in
ejaculation associated pain and a decrease in orgasm in the prostatitis group.
Conclusions : Our data have helped to elucidate the characteristics of symptoms in
prostatitis patients, and the questionaire can be used as a tool to standardize prostatitis
symptoms, and to evaluate treatment responses.
