Abstract
서론
전립선비대증은 50세 이상의 남성에서 가장 흔한 배뇨장애의 원인이며, 최근 국내에서도
식생활의 서구화, 노령층의 증가, 생활 수준의 향상 및 건강에 대한관심도가 증가하면서 그
중요성이 부각되어지고 있다.
전립선은 일반적으로 연령이 증가함에 따라 그 크기가 증가하고, 또한 전립선비대증의 유
병률도 증가하는 것으로 알려져 있다. 이러한 결과로 50세 ∼60세에 이르러서는 남성의 약
50% 정도가 병리학적 전립선비대증을 나타낸다. 전립선의 성장정도는 연령에 따라 차이가
있으며 서구인의 경우 10대부터 30대 사이에는1.6gm/year, 31세에서 90세까지는 점차로 성
장정도가 감소하여 0.4gm/year에 이르게되고, 전립선 크기가 두배가 되는 기간도 변화하여
30∼51세는 45년, 55∼70세는 10년, 70세 이후에는 100년 이상을 나타낸다. 이러한 결과는
서구인을 대상으로 한 역학조사를 통해 얻어진 것으로 한국인을 대상으로 전립선의 크기를
비교적 정확하게 측정한 역학조사는 없는 실정이다.
이에 저자들은 한국인의 연령에 따른 전립선 크기의 변화를 조사하여 이를 토대로 일반적
기준으로 이용할 수 있는 전립선의 크기를 정하기 위해 일정 지역의 지원자 645명에 대한
경직장초음파촬영술 및 직장수지검사를 시행하여 얻은 연령별 전립선 크기 및 분포, 정상군
과 전립선비대증군간을 비교하여 얻은 결과를 보고한다.

Purpose : It is known that size of the prostate increases with age, but there is no
guideline of size of the prostate to be applied to Korean men. This study reports the
size change of prostate with age in Korean men.
Materials and Methods : To determine the prostatic size, 645 volunteers(539 normal
group and 106 BPH group) from Chungbuk province underwent transrectal
ultrasonography (Scanner 200, Pie Medical, Netherlands). Size of the prostate was
estimated as a weight by the elliptical method.
Results : The mean prostate weight was 22.4±8.4gm(range 7∼83gm). The relationship
between prostate volume and age was statistically significant in 645 volunteers(r=0.17,
p<0.0005). Also, between normal group and benign prostatic hyperplasia group, there
was significant correlation with age(r=0.16, P<0.0005), while results were insignificant
with size(r=0.13, p>0.1).
Conclusions : Size and rate of growth of the prostate were smaller compared to the
western society, nevertheless, prostate size increased with age. For accurate and
reasonable age-specific size of the prostate of Korean male, community based
multicenter study will be needed with more accurate measurement techniques such as
step-section planimetry for the prostatic volume estimation. (Korean J Urol 1998; 39: 76
