부고환으로 전이된 전리선점액성선암 1례

Abstract
전립선암은 대부분 선암이며 점액성선암은 아주 드문 것으로 1881년 Boyd가 처음으로 전
립선점액성선 암을 보고한 후 전 세계적으로 약 50례가 보고되었으나, 2동일한 진단 기준이
결여되어 있으므로 그 신뢰성에는 의문점이 남아 있다.
최근 제시된 3가지 진단 기준을 적용하면 전립선점 액성선암의 발생률은 0.4%였으며, 전
이를 하는 장소는 골반림프절, 뼈, 뇌, 간, 폐, 방광 정낭 등이다. 전립선선암이 부고환에 전
이된 경우는 문헌상으로 약 15례가 보고되었으며, 일측 혹은 양측 모두 전이할 수 있으나
전립선점액성선암의 경우에는 보고된 바가 없다. 진단은 PAP, PSA, CEA 등에 대한 면역조
직화학반응을 시행하여 원발성 전립선점액성선암과 전립선으로 전이된 점액성선암의 진단이
가능하다. 예후는 전형적인 전립선선암과 비슷하거나 혹은 약간 나쁜 것으로 보고되고 있으
며, 종양을 차지하고 있는 점액 부분의 비율과는 관련이 없는 것으로 보고되고 있다. 그러나
증례의 수가 많지 않아 이러한 요인들에 의해 예후를 결정하기는 아직 미흡하다.
저자들은 전립선점액성선암이 좌측 부고환으로 전이된 1례를 치험하여 문헌 고찰과 함께
보고한다

Mucinous adenocarcinoma of the prostate is very rare tumor and comprises about
0.4% of all prostatic carcinomas. The metastatic form of mucinous adenocarcinoma of
the prostate is similar to that of the adenocarcinoma of the prostate. Common sites of
metastasis are pelvic lymph node and bone. At presentation, half of patients have
evidence of metastasis to pelvic lymph nodes, bone, brain, liver, bladder and seminal
vesicles. Especially, epididymal metastasis from mucinous adenocarcinoma of the prostate
is uncommon and only 15 cases was reported in the literature.
We report a case of mucinous adenocarcinoma of the prostate with metastasis to the
epididymis and discuss the clinicopathologic features of this lesion.
