Abstract
서론
최근 전립선암의 발생빈도는 평균수명의 증가와 식이습관의 변화에 의해 급격히 증가되고
있는 추세이다. 우리 나라에서도 전립선암 환자의 발생 빈도는 지속적으로 증가하고 있는
실정이며 치료에 대한 연구가 꾸준히 진행되고 있다. 전립선암이 내분비 의존성 질환임이
Huggins 등에 의해서 처음 밝혀진 이래로 전립선암의 치료에 있어서 호르몬 치료법의 개념
이 도입되었으며 그 결과 진행된 전립선암의 치료원칙으로 남성호르몬 박탈이 대두되었다.
남성호르몬 박탈의 방법으로는 외과적 제고술, 내과적 제고술, 표적세포에서 안드로젠 차단,
완전 남성 호르몬 차단, 2nd-line 내 분비 치료, 프롤락틴 억제, 5α-reductase 억제 등이 있
다. 외과적 제고술은 고환제거에 따르는 정신적인 충격 및 외관상의 문제점 등이 단점으로
지적되고 있고 그외의 내분비약물요법도 부작용과 경제적 부담의 문제점 등이 수반되고 있
다.
이러한 문제점을 보완하기 위해 저자들은 남성호르몬의 생성을 차단시키는 방법 중 외과
적 제고술을 시행하지 않고, 지금까지 보고된 바가 없는 무수알콜을 이용한 화학적 제고술
의 방법으로 웅성백서의 고환에 무수알콜을 주입하여 혈중 테스토스테론의 변화와 고환 및
전립선의 조직변화를 관찰함으로 외과적 제고술과 같은 효과를 얻고자 본 실험을 시행하였
다.

Purpose : In patients suffering from prostate cancer, endocrine treatment is commonly
applied to either locally advanced or metastatic prostate cancer. But, there are many side
effects with endocrine treatment in spite of its good response. We studied the effect of
castration using absolute alcohol injection into the testis of Sprague-Dawley rats.
Materials and Methods: We checked and compared the serum testosterone level after
surgical castration and the injection of a serial amount of alsolute alcohol into the testis
of sixty Sprague-Dawley rats. The histologic findings of the testes and prostates in
those injected with absolute alcohol were also evaluated.
Results: The testosterone level of normal control was 2.29±0.47ng/㎖ and that of the
bilateral orchiectomy group was 0.03±0.02ng/㎖. The testosterone level of the groups
that were injected with absolute alcohol more than 25% of testicular weight were the
same as the testosterone level of castrated rats. The histologic findings of the testes
and prostates in those injected with absolute alcohol more than 25% of testicular weight
were diffusely atrophied.
Conclusions: We suggest that chemical orchiectomy using absolute alcohol as a new
endocrine treatment is another modality in advanced prostatic cancer patients who need
an orchiectomy.
