Abstract
서론
맹단성요관류(cEcoureterocele)는 이소성 요관류의 매우 드문 변형으로 내강은 요관구의
뒤로 넓게 뻗어 있으며 요도의 점막하에 위치한다. 설상(tongue-like)혹은 맹장형
(cecum-like) 맹단이 특징으로 배뇨시 충만되어 방광출구 폐색을 일으키며 이로 인한 요역
류 및 요저류로 인한 반복적 인 요로감염과 폐색성 배뇨양상 등이 나타난다. 저자들의 문헌
조사에 의하면 1971년 Stephen이 처음으로 보고한 이후 현재까지 국내에서는 보고된 적이
없고 국외에서만 24례가 보고되고 있는 매우 드문 선천성 요로계 질환이다.
저자들은 산전 초음파에서 양측성 수신증과 함께 출산 후 요로감염의 발생으로 진단된 대
측 방광요관이 행부 협착을 동반한 맹단성요관류 1례를 치험하였기에 문헌고찰과 함께 보고
하는 바이다

We report a case of cecoureterocele with contralateral ureterovesical junction stricture
in 1 month-old boy who presented with bilateral hydronephrosis detected by prenatal
ultrasonography, Ultrasonography showed bilateral hydronephrosis, left complete duplex
system and lect ureterocele. Voiding cystourethrography demonstrated reflux into the left
upper moiety through ureterocele with prominent dilatation of the bladder neck and the
proximal urethra. Ureteral stricture at right ureterovesical junction was suspected by
intraoperative catheterization and retrograde ureteropyelography. Left upper pole
nephrectomy with partial ureterectomy, transvesical unroofing of cecoureterocele and
bilateral ureteroneocystostomy using Cohen's method were done. Convalescence was
unevenful and postoperative voiding cystourethrography revealed no evidence of
vesicoureteral reflux, dilated bladder neck or remnant cecoureterocele.
