Abstract
척수손상으로 인한 배뇨근-외요도괄약근부조를 나타내는 경우 대부분 항콜린성 약물과
자가도뇨법 또는 치골상부방광루설치술이 적용되지만, 이런 치료에도 불구하고 비교적 높은
방광내압으로 인하여 방광요관역류와 요로감염이 지속되거나 또는 심한 위축방광이 발생하
면 환자의 상태에 따라 영구적인 수술방법인 방광확대술 또는 외요도괄약근절개술이 선택되
고 있다. 그러나 방광확대술은 자가도뇨가 가능해야 한다는 점과 수술시나 수술 후의 합병
증이 문제가 되떠, 외요도괄약근절개술은 높은 실패율이 문제가 되고 있다. 저자들은 척수손
상 후 항콜린성약물과 자가도뇨법 치료가 적합하지 않았던 배뇨근-요도괄약근부조 환자 2
례에서 외요도괄약근부위에 요도부목설치술을 시행하여 만족할 만한 결과를 얻었기에 보고
하고자 한다.

In patients with spinal cord injury, detrusor sphincter dyssynergia is a common and
troublesome problem that may evoke progressive deterioration of upper urinary tract and
urinary tract infection including sepsis. Instead of external sphincterotomy, urethral
wallstent may be the useful treatment for the patients with detrusor sphincter
dyssynergia refractory to conventional treatment. In addition, this method has some
advantages of less invassive, less morbid and simpler technique than other treatment
methods. We report our preliminary results of the urethral
wallstent(MemothermR) for the treatment of 2 patients with complicated
detrusor sphincter dyssynergia.
