체외충격파쇄석기(Genestone 190)를 이용한 요석 환자의 치료경험

Abstract
결론
저자는 1995년 8월부터 1997년 2월까지 spark gap방식인 Genestone 190 기종으로 요석
환자 173례를 치료하여 그 결과를 분석하고 요석의 분쇄에 영향을 주는 인자를 조사하였다.
요석의 크기가 증가함에 따라 성공률은 감소하는 경향을 보였고 시술의 횟수는 많아졌으며
신석의 경우 20mm 이상에서 통계적으로 의미있게 증가했다. 요석의 위치에 따른 성공률에
서는 신배석의 성공률이 신우석과 다발성 신석의 성공률보다 통계적으로 의미있게 낮았다.
신석의 경우 누적성공률은 1회 47%, 2회 74%, 3회 86%이었고 요관석의 누적성공률은 1회
65%, 2회 79%, 3회 87%이였다. 시술 후 합병증으로는 육안적 혈뇨(161례), 산통(15례), 발열
(1례)등이 있었고 대중적요법으로 해결되었다. 석로현상은 18례에서 발생해 15례에서 자연배
출되었으나 1례에서는 체외충격파쇄석술, 2례에서는 요관경하배석술로 제거하였다. 이상의
결과로 볼 때 spark gap방식인 Genestone 190 체외충격파쇄석기를 이용한 쇄석술은 거의
모든 요석을 해결할 수 있고 쇄석술 후 발생한 합병증은 임상적으로 미미하여 요석 치료에
효과적이면서도 안전하고 용이한 방법으로 사료되며, 가격면에서 경제적이고 치료효과와 합
병증의 발생문제도 다른 기종의 쇄석기와 차이가 없었다.

Purpose: The authors evaluated of the efficacy of Genestone 190 lithotriptor (Genemed
company) for the treatment of the patients with urinary calculi.
Materials and Methods: From August, 1995 to February, 1997, Seventy patients with
venal stones and 103 patients with ureteral stones underwent SWL using fluoroscopy
targeting, under intramuscular analgesics. Characteristics of site and size of stones,
number of SWL sessions according to the size of stones, success rate according to site
or size of stones, and their complications were analysed.
Results: The cumulative stone-free rates of first, second, and third SWL session of
renal stones were 47%, 74%, and 86% and the cumulative stone-free rates of ureter
stones were 65%, 79%, and 87% respectively. As the size of urinary calculi increased,
the numbers of SWL sessions increased, but, the success rates were reduced. Success
rate of calyx stone was lower than that of pelvis and multiple stone. Post-SWL
complications were transient gross hematuria in 161 cases, steinstrasse in 18 cases, renal
colic in 15 cases, and fever in one case.
Conclusions: SWL monotherapy with Genestone 190 lithotriptor is a effective, safe and
useful treatment modality which can resolve almost all cases of urinary calculi without
any clinically significant complications.
