Abstract
요로결석의 치료로 현재 가장 많이 시행되어지고 있는 것은 요관경을 이용한 제석술과 체
외충격파쇄석술을 들 수 있다. 이들 술식은 개복수술에 비해 덜 침습적이며 술후 합병증이
적고 재원기간이 짧은 장점으로 인해 우선적인 치료방법으로 고려되어진다. 그러나 드물게
크기가 크고 오랫동안 지속되어 요관에 고착되어 있는 중위부나 원위부 요석의 경우 개복수
술의 적응증이 될 수 있다. 최근에는 침습적인 개복수술 대신에 복강경을 이용한 술기가 발
달하여 요관결석 제거술에서도 복강경을 이용한 제석술이 시도되고 있다.
복강경을 이용한 요관석제거술은 개복수술을 대신할 수 있는 치료방법으로 개복수술에 비
해 절개부위가 작고 덜 침습적이며 술후 합병증이 적고 재원기간이 짧은 장점을 가지고 있
다. 이 술식은 접근방법에 따라 후복막을 통한 방법과 복막강을 통한 방법으로 나눌 수 있
으며 모두 가스를 주입하여 공간을 확보하는 술식이 이루어져 왔다. 그러나 기존의
CO2 가스를 주입하는 방법은 laparoscopic port를 통하여 제한된 수술기구를
사용하기 때문에 술기 자체가 용이하지 않고 특히 결석제거 후 요관문합이 어려워 보편화되
지 못하고 있는 실정이다.
이에 저자들은 가스를 주입하지 않고 견인기구를 이용하여 복벽을 들어올리는 방법으로
원위부 요관석을 포함한 2례의 요관석에 대해 복막을 통한 복강경하요관석제거술을 성공적
으로 시행하였기에 수술술기와 함께 그 결과를 보고한다.

Laparoscopic ureterolithotomy has been attempted in the management of the large,
long-standing, impacted ureter stone. However it was not commonly used because of
difficulties in technique. So we report 2 cases of laparoscopic ureterolithotomy using the
gasless technique.
We performed the laparoscopic ureterolithotomy in two patients with ureter stones by
transperitoneal approach. We used the laparolift system which was connected by
laparofan retractor. The abdominal wall was elevated effectively, and the ureters were
exposed with ease. The stone was successfully removed in all two cases. The operative
time was 120 and 140 miniutes respectively. There was no intraoperative or
postoperative complication including urinary leak in all cases. Postoperative pain was
minimal.
This gasless technique is convenient and safe because an operator can use the
conventional surgical instruments and avoid the complications directly associated with
pneumoperitoneum.
