Abstract
거대요관-거대방광연합은 1954년 Williams가 커다란 용적, 부드럽고, 얇은 벽을 가진 방
광을 동반하면서 고도의 방광요관역류를 가진 환자에게 처음으로 적용하였다. 그 이후 이
질환이 주로 방광요관역류를 가진 어린 남아에서 나타나는 것이 발견되었고 병인을 방광자
체나 요도의 폐색으로 생각하여 용적감소 방광성형술(reduction cystoplasty)이나 방광경부
교정(bladder neck revision)을 시행하였다. 그러나 Harrow에 의해 병인이 방광 자체의 문제
보다는 고도의 방광요관역류인 것으로 밝혀져, 역류의 수술적 교정이 치료의 중점이 되어야
할 것으로 권장되었고 또한 높은 치료율이 보고되고 있다.
저자들은 고도의 방광요관역류를 동반하면서 거대 방광용적(750㎖)을 가진 소아에서 무기
능의 좌측 신장과 확장된 요관의 절제술후 정상방광 용적과 기능을 회복한 환자를 치험하였
기에 보고한다.
Megaureter-megacystis association describes the radiographic appearance of the large
capacity, thin-walled bladder and massive primary vesicoureteral reflux. The
pathophysiology of these massively dilated ureters and the bladder with large capacity
bases on the constant recycling of large volumes of a refluxed urine.
We report a case of megaureter-megacystis in 5-year-old girl who had large residual
urine(750 ㎖) with, massive ureteral reflux. After removal of left non-functional kidney
and dilated ureter, she has gained normal bladder capacity.
