방광에 발생한 육종양암 1례

Abstract
방광에 원발한 육종양암은 매우 드문 악성 종양으로 악성 상피세포 성분과 간엽조직 성분
이 섞여 있으며 방추형 세포가 뚜렷하게 나타나는 것이 특징이다. 일반적으로 암성분과 육
종 성분이 같이 있는 암육종과 비슷한 개념으로 여겨지고 있으며, 이들을 구분하는 것은 임
상적으로 의미가 없다. 조직학적으로 암육종에서는 간엽조직 성분으로 연골육종이나 골육종
이 흔하지만 육종양암에서는 대부분 방추세포로 이루어져 있다. 암육종은 주로 여성의 자궁
에서 발생하는 것으로 알려져 있으나 방광에 원발성으로 발생하는 경우도 간혹 보고되고 있
다.
육종양암 및 암육종은 성장속도가 빠르고 진단 당시에 주위 조직으로의 침윤된 경우가 많
으며, 대체로 항암화학요법 및 방사선치료에 저항을 보여 적극적인 치료에도 불구하고 예후
는 아주 좋지 않은 것으로 알려져 있다.
최근 저자들은 무통성 육안적 혈뇨를 주소로 입원한 69세 남자 환자의 방광 전벽에 발생
된 육종양암 1례를 경험하였기에 문헌고찰과 함께 보고한다.
Sarcomatoid carcinomas of the urinary bladder are very rare and highly malignant
tumors containing both malignant mesenchymal and epithelial elements. These exhibit a
prominent spindle cell component as a malignant mesenchymal elements and are tumors
with a poor prognosis despite aggressive treatment.
Recently we experienced a case of sarcomatoid carcinoma of the bladders in a 69
year-old male. He complained of painless gross hematuria and diagnosis was made by
immunohistochemical stain. Partial cystectomy was performed. After 5 months, he was
readmitted due to gross hematuria, voiding difficulty and generalized weakness. Pelvic
CT showed a recurrent mass in the bladder.
He received only conservative treatment and died 1 month later.
