5α-reductase 결핍에 의한 질성회음음낭부요도하열 1례

Abstract
한 개체에 고환과 난소가 동시에 존재하며 외성기가 남성인지 여성인지 모호할 때 진성반
음양이라 하고 고환 혹은 난소 중 한가지 만 존재하며 외성기가 모호할때 가성반음양이라
한다. 남성가성반음양은 윌러관의 퇴화가 안될 때, 테스토스테론의 합성이 부적절할 때, 디
하이드로테스토스테론의 합성이 부적절할 때, 또는 안드로젠 수용체의 결여에 의해 발생할
수 있다. 이 중 5α-reductase의 결핍으로 테스토스테론이 디하이드로테스토스테론으로 환
원되지 못하여 외부 생식기가 여성에 가깝게 되는 질환을 질성회음음낭부요도하열이라 하며
남성가성반음양의 약 13%를 차지하는 드문 질환이다.
본 교실에서는 여자로 자라 온 14세 환아에서 5α-reductase의 결여로 인한 질성회음음낭
부요도하열로 진단되어 수술로 치료하였던 1례를 경험하였기에 문헌고찰과 함께 보고한다.
Male pseudohermaphroditism can be caused by absent mullerian regression, inadequate
synthesis of testosterone, inadequate synthesis of dihydrotestosterone, or ardrogen
receptor deficiency. Defects in either the production or the action of androgenic steroids
knave been demonstrated to cause pseudovaginal perineoscrotal hypospadias, a syndrome
of male pseudohermaphroditism. This is mostly caused by a deficiency of 5α-reductase,
which controls the conversion of testosterone to 5α-dihydrotestosterone.
We report a case of male pseudohermaphroditism due to 5α-reductase deficiency who
was born with ambiguous genitalia and was reared as female.
