Abstract
방광암의 90% 이상은 이행상피암이며, 편평세포암은 3-7%를 차지하나 국내에서 복벽을
침범한 방광 편평세포암의 증례는 보고된 바 없다. 방광 편평세포암의 가장 성적이 좋은 치
료 방법은 물론 근치적 절제술이다. 그러나, 암세포가 복벽과 피부까지 침범한 경우 광범위
한 절제술은 복벽의 결손이라는 해결해야 할 또 하나의 문제를 안고 있다. 저자들은 복벽
및 피부까지 침범한 방광 편평세포암 환자에 대해 방광절제와 함께 광범위 복벽절제 후 대
퇴근막장근을 이용하여 복벽결손을 효과적으로 재건하였기에 수술 방법과 함께 증례 보고한
다.
Squamous cell carcinoma of bladder is best treated with radical cystectomy. Squamous
cell carcinoma of bladder invading the abdominal wall is not commonly reported and
hard to manage surgically. Abdominal wall defect caused by a wide resection of
squamous cell carcinoma invading abdominal wall can be reconstructed using various
muscle flaps. We performed a salvage cystectomy for a patient with squamous cell
carcinoma of bladder invading abdominal wall up to skin, and the abdominal wall defect
was successfully reconstructed with a unilateral tensor fascia lata myocutaneous flap.
