Abstract
이소성 요관은 요관이 방광삼각부의 측각부 이외의 장소로 개구하는 선천성 기형으로 약
80%에서 중복요관과 동반되지만, 단일이소성 요관은 이소성 요관 중 15-20% 정도를 차지
하는 드문 질환이다. 일반적으로 이소성 요관구의 개구부가 정상 요관구의 위치에서 떨어져
있을수록 신이형성 정도가 증가하며, 특히 단일이소성 요관에서 이형성신이 동반되는 빈도
가 높다. 이형성신은 기능이 없고 비정상적인 위치에 있는 경우가 많아 기존의 진단 매체로
는 진단 및 위치 파악에 어려움이 많다. 따라서 복부초음파검사나 배설성요로조영술, 배뇨중
방광요도조영술, DMSA 신주사, 방광경검사, 질경검사, 역행성요로조영술 등 여러 검사를
시행해야하고 수술적 개복 후에 진단되는 경우도 있다. 또한 신장의 위치 파악 및 이형성신
과 발육부전신을 감별하기 위해서는 복부 전산화단층촬영술까지 필요하다. 저자들은 기존의
진단 매체와 내시경 검사로 요관과 요관구의 위치 파악이 어려웠던 동측 기능부전신을 동반
한 단일이소성 요관 4례에서 MR urograpy를 시행하여 신우 및 요관을 영상화하고 요관구
의 위치를 확인함으로써 수술적 접근에 도움을 받았던 바, MR urography의 진단적 유용성
을 보고하고자 한다.
We report on 4 cases with a unilateral single ectopic ureter associated with ipsilateral
dysplastic or hypoplastic kidney and the usefulness of preoperative magnetic
resonance(MR) urography in revealing the ureteral course and detecting the insertion
site of ectopic ureter Among two cases of unilateral single ectopic ureter associated
with dysplastic kidney, one drains into the seminal vesicle and the other drains into th8
urethra. The other two cases have unilateral single ectopic ureter associated with
hypoplastic kidney that drain into the vagina. In all cases, MR urography shows the
ureteral course and insertion site of ectopic ureter regardless of the renal function. We
believe that MR urograhy is a useful technique for the diagnosis of unilateral single
ectopic ureter associated with hypofunctioning kidney.
