Abstract
방선균증(actinomycosis)은 혐기성 세균인 Actinomyces israeli에 의해 유발되는 만성 화
농성 염증이다. 골반강이나 요로 생식기의 방선균증은 대개 위·장관계 감염에 의한 이차적
인 감염, 또는 자궁 내 피임장치 사용과 동반된 경우 등이 다수 보고되고 있다.
저자들은 하복부 종물을 주소로 내원한 환자에서 요막관 종양이나 요막관 낭종의 감염으
로 의진하여 방광부분절제술을 실시한 후 방선균증으로 확진되었던 1례를 보고한다.
We present a case of urachal actinomycosis. Twenty four year-old female patient
presented with low abdominal pain and a palpable mass. MRI showed an irregular
shaped mass extending from the umbilicus to the dome of the bladder, just beneath the
rectus muscle. Exploration revealed a hard fibrotic mass which was adhered tightly with
appendix, dome of the bladder, left ovary, fallopian tube and sigmoid colon. The
ill-defined fibrotic mass with the adhered organ was removed as on bloc. Actinomycosis
was diagnosed from the postoperative specimen. Penicillin G was administered
postoperatively. At 1 year followup, she is well without subjective symptom except mild
frequency and lymphocele in right pelvis, which was aspirated.
