신석으로 오인된 신진주종 1례

Abstract
진주종은 비뇨기계 영역에서 비교적 드문 양성질환으로 현재까지 약 80례가 보고되고 있
다. 이중 Hertle과 Androulakakis의 보고에 의하면 진주종의 발생부위는 신우를 포함한 신
장부위가 64례로서 가장 호발하는 것으로 알려져 있다. 우리 나라에서는 고와 권이 1980년
도에 처음으로 신우 진주종을 보고하였다. 진주종은 구강내를 비롯한 기타 장기에서는 악성
변화를 보이므로 과거 비뇨기계 영역의 대부분의 증례에서는 악성변화의 가능성을 고려해
광범위 신적출술을 시행해왔으나, 근래에는 악성변화에 대한 확립된 증거가 없는 것으로 판
단되어 내시경술이 보존적 진단 및 치료로서 이용되고 있다. 본 증례에서는 간헐적인 좌측
복통으로 내원하여 제반검사상 좌측 신석이 의심되어 신우절석술 시행 도중, 요석이 아닌
석회화된 종물이 발견되어 결국 신적출술을 시행한 58세 여자환자에서 조직 검사상 비뇨기
계 영역에서 비교적 드문 질환인 진주종으로 확인되어 문헌고찰과 함께 보고한다.
Cholesteatoma of the upper urinary tract is a rare nonmalignant condition
histologically characterized by keratinizing desquamative squamous metaplasia(KDSM).
Until now about 80 cases including one case from Korea have been reported.
Cholesteatoma shows microscopic features of squamous metaplasia of the transitional
epithelium and keratinization. These changes with subsequent desquamation of the
superficial epithelial layers explain clinical manifestations of the disease. Flank pain,
passage of cornified material in the urine and a filling defect on the IVP constitute the
characteristic triad. Most cases were managed by extensive ablative surgery for fear of
malignant potential. We report a case of venal cholesteatoma which was misdiagnosed
as renal stone in 58 year-old female patient who had been suffered from intermittent
left flank pain.
