Abstract
1996년 Ulmsten 등1이 복압성요실금 환자에서 기존의 슬링수술의 방법과는 약간 다른 중부요도 슬링수술인 tension-free vaginal tape (TVT) 수술을 처음 소개한 이후 TVT 수술은 현재 여성 복압성요실금 치료를 위해 가장 널리 이용되는 수술 방법 중 하나이다. 현재까지 이러한 TVT 수술에 대한 단기 및 장기 치료성적들이 많이 보고되고 있고2-19 몇몇의 연구에서 절박성요실금이 동반된 복합성요실금, 낮은 복압성 요누출압, 고령, 골반강 수술의 과거력 등을 수술의 성공률을 떨어뜨리는 요인으로 보고하고 있다.9,11,15-18 이러한 TVT 수술 결과에 대한 보고들은 주로 수술 후 소변의 누출 유무에 따른 평가에 근거하고 있다. 그러나 국제요실금학회 (International Continent Society)20에서는 요실금 치료의 정확한 결과를 알아보기 위해서는 소변의 누출 유무 등의 증상에 관한 것뿐만 아니라 소변 증상이 환자의 일상생활에 미치는 영향도 함께 조사하여야 한다고 권고하고 있지만 아직까지 이에 대한 국내외의 보고는 미미한 실정이다.
Purpose:The factors affecting the success and quality of life following a tension-free vaginal tape(TVT) procedure were investigated in patients with stress urinary incontinence.
Materials and methods:We included 79 women with stress urinary incontinence that underwent the TVT procedure and were followed for at least 3 years. Preoperatively, the patients were evaluated by history, physical examination, a 1-hour pad test, and videourodynamics to determine the abdominal leak point pressure(ALPP). In addition, pre- and postoperative quality of life was evaluated by the incontinence quality of life questionnaire(I-QoL). We analyzed factors including patient characteristics, history, 1-hour pad test and ALPP with respect to the success and quality of life after the TVT. A success after the TVT was defined as the absence of any subjective complaint of leakage.
Results:The overall 3-year success rate of the TVT was 90%. There was a statistically significant increase in the I-QoL scores postoperatively. There were no significant preoperative factors affecting the success rate and the postoperative I-QoL scores. However, when the postoperative I-QoL scores were compared with the preoperative scores, the increase in the I-QoL scores was significantly higher in the patients with urge incontinence, low ALPP and high-grade incontinence.
Conclusions:The results of this study suggest that the TVT procedure is effective for treating female stress incontinence and improving the quality of life without any independent risk factors. However, for improving the quality of life, the TVT was more effective in women with stress urinary incontinence with urge incontinence, low ALPP and high-grade symptoms.
